0096771243 | 2013-11-22 | Production Wells | No Violations | I was on location with inspector Kevin Perry to investigate complaint #4545 about a gas smell and an oil leak. The inspection found that a flow line had formed a leak on the east side of the road where it runs to the tank battery on the west side. Several gallons of oil had leaked out onto the grass and into a ditch. The impacted area was was approximately 15 feet long and several feet wide and had been covered with peatsorb. I spoke with the landowner and Kevin contacted Enervest and requested that the impacted area be cleaned up immediately. Enervest field personnel arrived on location to address the release and were waiting on a dump truck and excavator to remove the impacted soil. The well was shut in the previous day after discovering the leak. No waters of the State were impacted by the release and inspector Perry will be overseeing the clean up. No violations. |

1107434545 | 2015-10-15 | Production Wells | No Violations | I conducted a status check with DOGRM Regional Supervisor Tom Hill. The well is equipped with an electrically-driven pumpjack, and the production valves were in the open position. Proper identification was posted at the well and tank battery. The tank battery pad is shared with G&O tank battery PN#(20463) and consists of one- |
1377709096 | 2013-08-22 | Production Wells | No Violations | I was on site with Helen Halter, my inspection found the well in production and in good repair. The well is a pump jack well. No identification was posted on the well. I contacted Enervest and requested an identification sign be placed on the well . |
1385389863 | 2013-11-21 | Production Wells | No Violations | I was on location with Mike Dinsmore in regards to a landowner complaint #4545. There was oil coming from the ground about 5 ft in from the road across from the tank battery. The flow line had rusted through and was leaking oil. The spot on the ground was 3ft by 5ft long and then went into the ditch. The ditch was not flowing with water so the oil was only in a 5 ft section of the ditch. An Enervest crew was on site performing the clean up and fixing the leaking flow line. |
1386594570 | 2013-12-04 | Production Wells | No Violations | My inspection found the contaminated soil has been removed from the location. There is still a 5 ft by 2 ft area approximatly 2 ft deep, that needs filled in. The company had severed a phone line while digging up the flow line to repair it. The phone line has been repaired and Tim McConaha from Enervest is sending a crew out to close the hole. |
