2578889 Primary Facility 2017-04-03 Routine/Partial Inspection No Violations Noted On 4/3/17 at 1347 hrs I attended a preconstruction meeting for the Sweden Valley 106 well site. The following people were present: JKLM Energy representative, Jeff Long; DCNR Gas Forester, Mike Cole; and J.L. Watts representative, John Watts. Penn E&R developed the Erosion and Sediment (E&S) Control plan and J.L Watts is the construction contractor.

We reviewed the E&S drawing. Site drainage is to Nelson Run, which is designated as a High Quality stream. Approximately 8,150 feet of a pre-existing haul road will be improved to serve as the access road to the well site. The access road entrance will come off the Billy Lewis road. The site will consist of a well pad (approximately 350 feet x 500 feet) and two sediment traps; Sediment Trap 1 (ST-1) will be along the north side of the pad and Sediment Trap 2 (ST-2) will be along the west side. The pad will be soil cemented for stability and sumps will be installed in the northwest and southwest corners. The well cellars will be located within a prefabricated st
