2515067 Primary Facility 2016-08-26 Routine/Complete Inspection No Violations Noted Arrived @ 10:30. Weather: cloudy. This is a routine inspection. There were no personnel onsite. The 1H well was in production. The cellar was empty.

Top gauge: 1100 psi,

5-1/2” : 1100 psi. 

13-3/8” x 9-5/8”: 0% /volume combustible gas 9-5/8” x 5-1/2” 0%/volume combustible gas (Altair 5X calibrated on 08/26/16). Left location @: 11:30.

2560080 Primary Facility 2017-02-08 Routine/Complete Inspection No Violations Noted Routine inspection to assess overall site conditions. No Chief representatives were present.

Overall vegetative cover is greater than 70% except for areas with extremely rocky soil conditions. Temporary erosion and sediment control BMPs are still in place. No evidence of accelerated erosion or sedimentation was observed.

According to the Department of Environmental Protection's records, a request for a 2-year extension of time for site restoration was submitted on February 16, 2016. To date this request has not been approved.

No violations were cited during this inspection.

2680131 Primary Facility 2018-01-09 Routine/Complete Inspection No Violations Noted Arrived on site at 11:30. Weather: blue sky and clouds some wind. To Wells on location. The B1H was in production. Top gauge: 1000 psi (02 5000 psi, analog). Annular Vents were shut-in.

The B2 H had a frac head and was shut in. There were seven Baker tanks on site near the wells. These were on containment. No violations were observed at the time of my inspection.

2707768 Primary Facility 2018-03-15 Routine/Complete Inspection No Violations Noted Arrived at 8:45. Weather: cold and sunny snow on the ground. The purpose of this inspection is to conduct an in active well inspection. There are two wells on the pad. Th The B1H well is in production. The other well regulatory in active status application.
2723040 Primary Facility 2018-04-25 Routine/Complete Inspection No Violations Noted The B1H well is in production. The other well regulatory in active status application. One dehydration unit in operation. No leaks from production equipment.
2788039 Primary Facility 2018-10-11 Routine/Complete Inspection No Violations Noted A well site restoration extension request was submitted to the Department on 9/6/18.

It had been raining throughout the day and some puddles had formed on the pad surface. Multiple conductivity screenings were taken and all had results below 110 µS/cm. The site was >70% vegetative cover and no E&S issues were observed.

There are 2 properly tagged Marcellus wells staged for production. There are 2 GPUs and 1 steel produced water tank onsite and in secondary containment. No corrosion was noted the the produced water tank and all secondary containment appeared to be in good condition.

At the time of inspection the pad remained at its original size and it appeared that all PCSM BMPs had been installed as depicted on PCSM plans and were functioning correctly.

No violations noted.

The Department recommends acknowledging the extension request.
