|  The Department traveled to the Reber-Dozier Well Site to conduct a Routine Partial Inspection.  At the time of the inspection, the Site ID Sign was present and the E&S Plan was on site.  The E&S Inspection Log indicated that the last inspection that was conducted took place on 1/21/15 by Randy Haig who indicated that the site was snow covered.

| The Department was on site at the Reber-Dozier Well Site to conduct a Routine Complete Inspection.  At the time of the Department's inspection, it was found that a review of the E&S Plan and E&S Inspection Log could not be conducted because the Department was unable to access them due to the height of their placement.  The site remains idle with no apparent activity.  While on site, the Department left a voice message with Mike Deming (SWN) concerning a damaged  tote that was found situated near the cut slope.  Holes were drilled in this tote and the fluid level was approaching the holes.  Operator, please investigate the origin of the tote and dispose of it properly.  Additionally, please survey the surrounding area to determine if a release occurred.
