Tubing was being installed in the wells today.

Site ID sign was in place and permits and plans were in the box.  Access road was in good shape and the site was guarded.  The site is undergoing completions.  Crews are currently plumbing in the 6 heater units and the 3 tanks which are in containment.  The sales line has been installed.
 
The impoundment on site had over two foot of freeboard on this date.
 
There are three sed traps on site and all looked to be functioning.  All three have old vegetation in them.
 
The site is stable with good plant growth.
 
The extension is hereby approved until the new restoration date of 3/20/2016.

The site overall was well vegetated. The purpose of the site visit was to meet with Mr. Blaney, landowner of part of the area permitted, and Nick Staffieri of Chevron to discuss Mr. Blaney farming a portion of the area under permit to the north east and east of the pad. The area has traditionally been farmed and there were no waters of the Commonwealth present in the immediate area or downslope of the area to be farmed observed. Approval was granted to Mr. Blaney to begin farming his land.

Since the site was constructed, a landowner constructed a home and graded the land adjacent to the access road to the south east. Chevron recently installed a drainage channel and rain garden on the opposite side of the access road and were to begin construction of the channel of interest when the landowner voiced their concerns that they did not want the channel installed.

2252555 Primary Facility 2014-03-11 Routine/Complete Inspection No Violations Noted On 3/11/2014, the Department conducted an inspection at the Chevron Appalachia's Rosa 6H - 11H well site in Franklin Twp., Fayette County. The reason for the inspection was a request by the operator for an extension on the date that the site needed to be restored by. The original restoration date was 3/20/2014 and an extension to 3/20/16 is being requested.

2487019 Primary Facility 2016-05-05 Routine/Complete Inspection No Violations Noted On 05/05/16 at approximately 0730 hours I conducted a routine complete inspection of the Rosa Unit. The Act 9 sign was in place and visible. All required documents were present in the mail box. The site overall was well vegetated. There were no violations observed at the time of inspection.
2487044 Primary Facility 2016-05-13 Routine/Complete Inspection No Violations Noted

On 05/05/16 at approximately 1300 hours I conducted a routine complete inspection of the Rosa Unit. The Act 9 sign was in place and visible. All required documents were present in the mail box. The site overall was well vegetated. The purpose of the site visit was to meet with Mr. Blaney, landowner of part of the area permitted, and Nick Staffieri of Chevron to discuss Mr. Blaney farming a portion of the area under permit to the north east and east of the pad. The area has traditionally been farmed and there were no waters of the Commonwealth present in the immediate area or downslope of the area to be farmed observed. Approval was granted to Mr. Blaney to begin farming his land.

2566327 Primary Facility 2017-03-01 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present. The site was well vegetated and there were no signs of erosion observed. Despite heavy rains, stormwater controls appeared to be properly functioning. The on pad equipment appeared to be in good condition. There were no violations observed at the time of inspection.
2584848 Primary Facility 2017-04-20 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The site was well vegetated and there were no signs of erosion observed. BMPs appeared to be well-maintained. The production equipment appeared to be in good condition. There were no violations observed at the time of inspection.
2603172 Primary Facility 2017-06-12 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place, but the E&S plans reflecting the major mod were not present on site. I contacted Chevron and a set of plans will be placed on the site. The site was well vegetated except the recently disturbed locations. PCSM had recently been added to the site and the disturbed areas appeared to have been seeded and mulched. There were no signs of erosion observed at the time of inspection.
2615372 Primary Facility 2017-06-30 Routine/Complete Inspection No Violations Noted Representatives of Chevron requested an on site meeting at the location to discuss the installation of a drainage channel. Since the site was constructed, a landowner constructed a home and graded the land adjacent to the access road to the south east. Chevron recently installed a drainage channel and rain garden on the opposite side of the access road and were to begin construction of the channel of interest when the landowner voiced their concerns that they did not want the channel installed.

The location where the channel is to be installed is well vegetated and there were no signs of erosion observed. Chevron is seeking guidance if the channel could not be installed and the plan amended by red line changes or if a major or minor modification would need to be submitted to DEP Environmental Engineering staff. This WQS is requesting guidance from Department Environmental Engineering staff regarding the issue. Once a determination has been made Chevron will be notified.

2682063 Primary Facility 2018-01-10 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The site was well vegetated and there were no signs of erosion observed. The production equipment appeared to be in good condition. There were no violations observed at the time of inspection.

The production equipment appeared to be in good condition. There were no violations observed at the time of inspection.
2771488 Primary Facility 2018-08-27 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The site was well vegetated and there were no signs of erosion were observed. The access road showed signs of washout. It is recommended the road receive maintenance.

Installed PCSM features appeared to be in good condition. The roadside ditch along the right side of the access road adjacent to a property owner's yard was still not installed. The property owner requested the ditch not be installed to allow them to mow their yard more easily. Chevron is currently preparing a modification to submit to the Department to address the issue.

The production equipment appeared to be in good condition. There were no violations observed at the time of inspection.
