|  On April 8, 2010 I conducted an inspection for an OG 57 Request for waiver of distance requirement. The well site is proposed to be constructed within 100 ft. from an unnamed tributary to Little Plum Creek. Well was site staked. Construction has not begun at site. Proposed E & S plan should protect stream from adverse impact during site/well development if properly implemented & maintained.  Drill cuttings are to be land farmed on site. Operator must ensure that the surface of the land farm area is at least 20 inches above the seasonal high groundwater table.

The well site is proposed to be constructed within 100 ft. from an unnamed tributary to Little Plum Creek. Well was site staked. Construction has not begun at site. Proposed E & S plan should protect stream from adverse impact during site/well development if properly implemented & maintained. Drill cuttings are to be land farmed on site. Operator must ensure that the surface of the land farm area is at least 20 inches above the seasonal high groundwater table.
1885579 Primary Facility 2010-05-19 Routine/Complete Inspection No Violations Noted On May 19th, 2010 I conducted a routine inspection of the above site. Drilling was taking place at the time of the inspection. A site ID sign and E&S plan were posted at the site. The access road was in good condition. A mud broom waas on site to keep the public roadway clean. All E&S controls were in place and constructed according to the plan at site.

The operator should continue to inspect the E&S controls routinely and after each significant runoff event until the site is restored.
