| I met with Dave Bartholomew of Nu Weld Incorporated.  Mr. Bartholomew informed me that Nu Weld is currently in the process bringing up flow risers to the well heads.  Natural gas will flow through risers/flow lines to the production units at the site.  The 1H, 2H, 3H and 7H wells have been recently drilled to total depth and the 4H, 5H and 6H wells were in production, but are currently shut in according to Mr. Bartholomew.  I observed dark staining impacting the pad surface next to around the perimeter of the mousehole/rathole and also impacting the well cellar grates.  The dark material appears to be oil based mud.

| I met with Brad Campbell, Field Superintendent of Talisman as I arrived at the location.  According to Mr. Campbell flowback of the Bennett 1H, 2H and 3H wells are in progress, with Flowback operations commencing on 9/10/15.  Frac plugs were drilled out on 8/31/15.  OPSCO Energy Industries is the contractor conducting flowback operations.  It appears as if fluid has leaked out of the bottom of Vacuum Box tanks tanks onto containment as indicated in the attached photos.  The fluid has a rust like appearance to it as it is orange, brown and red in color.  I did not observe any evidence that the fluid, which was elevated in conductivity,  escaped containment and impacted the ground surface of the pad.

| It appears as if all of the wells on the location are in production.  The Gas Production Units appear to be fully operational.  During the inspection a vacuum truck mobilized to the location and commenced the process of removing production fluid from the production fluid tanks.  I observed (2) small petroleum sheens, rainbow-colored on containment near the wells, (1) at N-41.79968°; W-076.12882°, the other at N-41.79948°; W-076.12888°.  I used a Hanna Soil Test and Electrical Conductivity & Temperature meter and Oakton Multi-Parameter Tester 35 to field screen water on and off containment and the pad surface.  The meter recorded (6) milliSiemens/cm conductivity on containment in the frac tank battery were the tanks have been removed.  Conductivity readings were in the range of (10-11) milliSiemens/cm in on the pad surface at N-41.79984°; W-076.12924° in water accumulated on the pad surface next to the frac tank battery containment where frac tanks used during flowback, remain.  This appears to be the area where trucks offload fluid from frac tanks

| All wells on-site are set up for production, but it appears as if the 3H, 4H, 5H and 6H wells are on-line, while the 1H, 2H and 7H wells are shut are in.  A frac tank staged in a containment system is plumbed from the wells to apparently contain sand and possibly production fluid from the wells. It appears as if the 4H, 5H, and 6H wells have been recently re-painted or coated with a fireproofing material.  There are chemicals apparently used in this process staged on plastic sheeting, without a berm around the perimeter of it.  There is an Atlas Copco air compressor used in this process staged on the pad surface with no secondary containment under it.  A bag of New Age Blast Media has been ripped open and a portion of the material in the bag has been spilled to the pad surface.   I observed a small pool of a red-dyed fluid indicative of diesel fuel spilled in containment used for a light tower next to the pad berm near the company man trailer and close to the pad entrance and access road.

| On December 27, 2017, the Department conducted a routine inspection at the Repsol Oil and Gas USA LLC, Bennett R 05-164 well site in Stevens Township, Bradford County.  The Department was represented by Andrew Hobbins.  A Repsol employee was on site conducting an inspection while the Department was on site.

The site sign, E & S plan and weekly inspection logs were on site and available for review.  The site had a dusting of snow covering the ground, but the site appeared to be well vegetated.  The well pad has not been reduced in size, but PCSM has been addressed.

Seven wells were in production during the inspection.  The wells were labeled and no bubbling was observed in the well cellars.  Two production tanks and seven GPUs were on secondary containment.  The puddles on the pad were all frozen, so conductivity readings were taken.  No staining was observed on the well pad surface.

I observed a small pool of a red-dyed fluid indicative of diesel fuel spilled in containment used for a light tower next to the pad berm near the company man trailer and close to the pad entrance and access road.

2675773 Primary Facility 2017-12-27 Routine/Complete Inspection No Violations Noted On December 27, 2017, the Department conducted a routine inspection at the Repsol Oil and Gas USA LLC, Bennett R 05-164 well site in Stevens Township, Bradford County. The Department was represented by Andrew Hobbins. A Repsol employee was on site conducting an inspection while the Department was on site.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site had a dusting of snow covering the ground, but the site appeared to be well vegetated. The well pad has not been reduced in size, but PCSM has been addressed.

Seven wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. Two production tanks and seven GPUs were on secondary containment. The puddles on the pad were all frozen, so conductivity readings were taken. No staining was observed on the well pad surface.

No violations were observed during the inspection.

2724908 Primary Facility 2018-05-01 Incident- Response to Accident or Event No Violations Noted Warm/windy at time of inspection.

Reason for this inspection is Repsol reported to the department that there was an increase in production water on the 4H and 5H caused by a communication of wells during the frack process on SWN's Seymour 4H well.

Please note there was no environmental incident, loss of well control, or other well bore integrity issues related to the increase of water production.

At the time of this inspection there was no indication of increased methane flow from the Intermediate x Production annulus vent. There were no other vents present at time of inspection.

2814364 Primary Facility 2018-12-12 Site Restoration No Violations Noted NOT approval.

No electrical conductivity readings taken from pooled water, because it's all frozen today. Air temp: 32F. JLP
