Work was being done on Folta N Bra 2H and then would be done on the Folta S Bra 2H before the NOMAC 75 arrived onsite to drill more wells. Further work planned was to also remove packer and tubing. This would require the workover rig and snubbing unit (from Makayla). Vince Engbith was also on site to lay down containment in prep for NOMAC 75. Left at 11:45.

| Nomac Drilling Rig #75 is currently in the process of drilling the vertical section of the Folta S Bra 1H at a depth of (3,609) feet.  In the backyard area, I observed residue on the containment covering the site perimeter berm and vegetation on the top of the berm at the top of the site cut slope.  The residue is grayish in color and appears to be drill cuttings mixed with lime as part of the Pug mill process, either overtopping the containers storing the cuttings or the cuttings being transported out of the containers by wind.  Lime is utilized as a drying agent for the cuttings. Fluid levels in (2) containers on containment in the backyard appear to exceed the (2)’ freeboard requirement of Title 25, Env. Protection, Pa. code 78.56.  The containers contain storm water accumulated on containment and drilling wastewater according to Mr. Hooper.

| I conducted an inspection of the site accompanied by Andrew Hobbins, DEP O&G WQS of NCRO in Williamsport.  Chesapeake submitted a stimulation notice to the Department on 9/16/16.  The notice indicated that CHK would be moving to the Folta Pad between 6:00 AM and 12:00 PM from 9/19/16 to 9/20/16.  The Folta S Bra 1H, Folta S Bra 3H, Folta N Bra 3H and Hattie N Bra 3H wells will be stimulated.  A coil tubing unit is on the location.  We met with Ashley Adcock and Tim Long, completions consultants of Chesapeake.  Mr. Adcock and Mr. Long stated the following. "The coil tubing unit is in the process of cleaning out sand in the Folta S Bra 1H well.  The well has had some issues after the frac was completed.  Frac operations will resume when the sand cleanout of the well is completed.  The secondary containment is Jersey Barriers, Rig Mats with A (30) mil liner below the mats and a layer of felt below the liner.  There are no wells in production. They are shut in.  The Folta N Bra 2H and S Bra 2H were in production

| On September 18, 2017, the Department conducted a routine inspection at the Chesapeake Appalachia LLC, Folta Location in Tuscarora Township, Bradford County.  Andrew Hobbins represented the Department.  Nobody was present for Chesapeake.

For the Folta N Bra 2H, the wireline was setting plugs inside tubing using a lubricator and wireline BOP. At the time of my inspection, wireline was setting the first plug and vac truck was vaccing puddles on the pad. 2391494 Primary Facility 2015-07-02 Routine/Complete Inspection No Violations Noted Nomac Drilling Rig #75 is currently in the process of drilling the vertical section of the Folta S Bra 1H at a depth of (3,609) feet. In the backyard area, I observed residue on the containment covering the site perimeter berm and vegetation on the top of the berm at the top of the site cut slope. The residue is grayish in color and appears to be drill cuttings mixed with lime as part of the Pug mill process, either overtopping the containers storing the cuttings or the cuttings being transported out of the containers by wind. Lime is utilized as a drying agent for the cuttings. Fluid levels in (2) containers on containment in the backyard appear to exceed the (2)’ freeboard requirement of Title 25, Env. Protection, Pa. code 78.56. The containers contain storm water accumulated on containment and drilling wastewater according to Mr. Hooper.

Chesapeake submitted a stimulation notice to the Department on 9/16/16. The notice indicated that CHK would be moving to the Folta Pad between 6:00 AM and 12:00 PM from 9/19/16 to 9/20/16. The Folta S Bra 1H, Folta S Bra 3H, Folta N Bra 3H and Hattie N Bra 3H wells will be stimulated. A coil tubing unit is on the location. We met with Ashley Adcock and Tim Long, completions consultants of Chesapeake. Mr. Adcock and Mr. Long stated the following. "The coil tubing unit is in the process of cleaning out sand in the Folta S Bra 1H well. The well has had some issues after the frac was completed. Frac operations will resume when the sand cleanout of the well is completed. The secondary containment is Jersey Barriers, Rig Mats with A (30) mil liner below the mats and a layer of felt below the liner. There are no wells in production. They are shut in. The Folta N Bra 2H and S Bra 2H were in production

The four wells drilled in 2015 had recently been fracced. All were shut-in at the time of my inspection. Onsite were workers with ESSi Corp, AMI, and Control Tech. Spoke with Tad Gonzales (Essi Corp) who was installing remote instrumentation at the time of my inspection. For this inspection, accessible annular vents were monitored using an Altair 5X gas meter calibrated on 12/6/16.

2637656 Primary Facility 2017-09-18 Routine/Complete Inspection No Violations Noted On September 18, 2017, the Department conducted a routine inspection at the Chesapeake Appalachia LLC, Folta Location in Tuscarora Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for Chesapeake.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was stabilized with a combination of rock and vegetation. No erosion issues were observed. PCSM has not been addressed at the site.

Six wells were in production during the inspection and all six were labeled. Six GPUs and two production tanks were also on site. Puddles on the well pad surface were low in specific conductivity. No staining was observed.

No violations were observed during the inspection.

South 2H: top gauge: 1541.8 psi (digital), North 2H: top gauge: 1230.4 North 3H, top gauge: 1380 psi, minor bubbling at threads of 2" bull plug on surface casing dump line. Verifiedmethane with Altair 5X gas meter and called to inform Eric Hottenstein. Hattie N Bra 3H: 2390 top gauge, Folta S Bra 1H: couldn't read top gauge.,Folta S Bra 3H: couldnt read.

2787900 Primary Facility 2018-10-05 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection. The areas around the well pad were well vegetated or rock lined. No E&S issues were observed.

There are 6 properly tagged Marcellus wells staged for production. There are 6 each GPUs and sand separators onsite and in secondary containment. There is 1 double walled fiberglass produced water tank onsite. It may be advisable to maintain a more traditional secondary containment to avoid issues while emptying produced water tanks.

PCSM/SR plans depict 2 compost filter sock (CFS) sediment traps on the east side of the pad (Photos #2, 3). At the time on inspection some water was being retained in these areas but no CFS was evident. An email was sent to Eric Haskins to check the status of PCSM.

An email received from Eric on 10/11/18 stated that PCSM has not yet been constructed. Chesapeake is currently addressing uncompleted PCSM on several sites.
