2695482 Primary Facility 2018-02-12 Routine/Partial Inspection No Violations Noted At the time of inspection, a welder was onsite welding well plates with well numbers on to each well. 16" conductor pipe has not been cemented in yet, but pipe is at 40' in depth. Cellar was set. Pad was snow covered. Well was open and awaiting cementers and will be covered afterwards. There is still construction activity just off of the pad, along the lease road.
2706919 Primary Facility 2018-03-19 Routine/Partial Inspection No Violations Noted On 3/19/2018, WQS Brian MacQuarrie onsite. Site sign installed and maintained at entrance. Site constructed with exception to freshwater impoundment. Erosion and Sedimentation best management practices installed. Site temporarily stabilized with seed and mulch. Drilling commenced at six locations on the well pad ( 1H-Permit No. 37-019-22632, 2H- Permit No. 37-019-22656, 3H- Permit No. 37-019-22657, 4H- Permit No. 37-019-22658, 5H- Permit No. 37-019-22659 and 6H-Permit No. 37-019-22660. Steel plates and cages installed at each location.

A thunderstorm was going through the area. Surface casing had been cemented using 315 sxs Class A (67 bbls), 1% CaCl2, 1% Gypsum, 15.8 ppg, 1.2 yield. This was displaced with 63 bbls freshwater and plug was bumped. Floats held. 14 bbls of cement returned to surface. Intermediate casing information was emailed later in the day and it had been cemented using 770 sxs Class A (163 bbls), 0.5% CaCl2, 1% Gypsum, 15.8 ppg, 1.19 yield. This was displaced with 174 bbls freshwater and plug was bumped. Floats held. 39 bbls of cement returned to surface.
2763621 Primary Facility 2018-08-14 Routine/Partial Inspection No Violations Noted On 8/14/2018, WQS Brian MacQuarrie onsite. Site sign installed at entrance . Erosion and Sedimentation plan onsite. Freshwater impoundment excavated and fenced in not lined. Two infiltration basins installed. The well pad consists of six wells partially drilled Permit Nos. 019-22632, 019-22656, 019-22657, 019-22658, 019-22659 and 019-22660. Containment liner setup around wells.
