2466442 Primary Facility 2016-03-21 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 11:20 a.m. for a new spud inspection, Lighting Energy was rigged up over the 5H and was drilling the hole for the conductor pipe. I met with equipment operator Adam Matheny, the conductor pipe was set and cemented on the 12H and 10H at 130'. Because of water in the holes, the holes were drilled to 60' on the 6H, 8H, 4H, 9H, 7H, 1H, and 3H, when they get to 60' on the 5H they plan on going back to each hole and drilling to 130' and setting the conductor pipe. I took a walk around the location and everything looked OK.

I left the site at 11:45 a.m.

2530363 Primary Facility 2016-10-11 Routine/Complete Inspection No Violations Noted Patterson Drilling Rig #805 is currently on the location. The current drilling depth is 1,625 feet. The most recent Drilling Resumption notice After Break of 30 days or more was submitted to the Department by Talisman on 10/7/16. The notice indicated that Talisman would commence drilling of the Kropp 6H well on 10/10/16 at 12:01 PM. I met with Vaughn Swanson, site company man of Talisman and Tory Rogers, site safety man of REM Security. Mr. Vaughn informed me the following information. "We are in the process of running a cement bond log. The intermediate casing will be run today and cemented later today. We are drilling with water based mud through the Upper Devonian formation. The surface casing was cemented at (562) feet. We will drill with water based mud through the kickoff point at (4,800) feet. Soli-bond is used to dry the cuttings instead of sawdust or wood pellets. 2540490 Primary Facility 2016-11-29 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 12:50p.m., I met with company man Vaughn Swanson, Patterson Rig 805 was on site and drilling the hole for the production casing. While cementing the surface casing, they lost circulation when they were running there displacement, they topped out with 10bbls of cement. I took a walk around the location and there were no obvious concerns noticed.

Depth at time of inspection was 5,715'. Projected TD is 11,700' I left the site at 14:00p.m.

2551253 Primary Facility 2017-01-06 Incident- Response to Accident or Event No Violations Noted Jack Burns, company man of Talisman Energy contacted David Schoonover, DEP OGI at 8:15 AM on 1/6/17 to report a spill of (2) gallons oil based mud to the pad surface. I arrived at the site at 12:30 PM on 1/6/17 to conduct an investigation of the incident. I met with Tory Rogers, site safety man of REM security. Talisman was in the process of breaking down the Patterson number 805 drilling rig and equipment and moving it off-site. Mr. Rogers stated the following information associated with the spill. "The spill occurred yesterday, 1/5/17, around 9:00 AM. (2) gallons of synthetic oil based mud from a flow line from that runs from the rig to the mud tanks was spilled to the pad surface about 60 to 70 yards from the where the lease road and well pad meet. The pipe was loaded onto the back of a flatbed truck and rolled as it was set spilling the mud out of the pipe onto the ground. There was no bull plug on the end of the pipe. The spill was cleaned up immediately. A bull plug was attached to the pipe.
2556749 Primary Facility 2017-01-20 Incident- Response to Accident or Event No Violations Noted Frank Hoehle, DEP O&G WQS and I arrived at the site at 11:40 in response to a reported spill of (170) gallons of hydraulic fluid to secondary containment. On 1/15/17, Kevin Augustine, Emergency Response Coordinator of the PADEP NERO Emergency Response Team contacted Eric Rooney, Licensed Professional Geologist in the Oil and Gas Management Program in the Scranton District Office to report the spill. The long description of the standard detail report states the following. "Bill Watts of Talisman notified Kevin Augustine (NERO - ER) who then referred spill to Eric Rooney (Scranton Office - O&G) that 170 gallons of hydraulic fluid was spilled onto 2nd containment." We met with Dave Coxhead, site safety man of REM security upon arrival to the location. Mr. Coxhead stated that Repsol was in the process of rigging up frac operations and they may start hydraulic fracturing of 10 wells here tomorrow. We had a brief discussion of the reported hydraulic fluid spill with Mr. Coxhead. He informed us that he was no
2562369 Primary Facility 2017-02-15 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:20a.m., met with company man Dell Gunn and safety coordinator Dave Coxhead. Halliburton was on site to frack the 1H, 3H, 4H, 5H, 6H, 7H, 8H, 9H, 10H, and 12H wells. They were fracking stage 18 out of 20 on the 5H and running a plug and perf in the 3H. They were doing a zipper frack and doing the odd numbers first. Fresh water was being pumped to the site from the Ayers impoundment and recycled water was being trucked onto the site. I took a walk around the location and there were no obvious concerns noticed.

There were 22 stages out of 25 completed on the 1H. There were 15 stages out of 18 completed on the 3H. There were 14 stages out of 16 completed on the 7H. There were 14 stages out of 17 completed on the 9H. I left the site at 11:05a.m.

2568858 Primary Facility 2017-02-23 Incident- Response to Accident or Event No Violations Noted Frank Hoehle, DEP O&G WQS and I arrived at the site at 11:30 to conduct an investigation of an diesel fuel spill of an estimated 1-5 gallons to the ground surface and one-half gallon of antifreeze onto Rig Mats. Halliburton is the Repsol subcontractor performing hydraulic fracturing operations. We met with Gordon Bennett, site safety man of REM security and Sheridan Anderson site company man of Repsol. Mr. Bennett and Mr. Anderson stated the following in reference to the spills and site activities. "Stimulation is in progress on the 8H well. Zipper frac is also in progress with 5 wells completed.

Antifreeze spill happened on 2/22/17. Less than one gallon of it was spilled onto rig mats from a coolant hose on a truck. This sand truck finished blowing off sand and noticed the leak. It was immediately soaked up with absorbent material. The diesel fuel spill of approximately one gallon occurred on the lease road on 2/20/17. 2572441 Primary Facility 2017-03-06 Incident- Response to Accident or Event No Violations Noted Frank Hoehle, DEP WQS and I arrived at the site at 11:30 in response to 2 separate spill incidents reported to the Department. We met with Sheridan Anderson, site company man, completions consultant of Repsol and Gordon Bennett, site safety man of REM security. Halliburton is the Repsol subcontractor conducting hydraulic fracturing operations. Mr. Anderson informed the Department he had reported both spills to Bob Bisignani, Assistant Emergency Response Coordinator of PADEP Northeast Regional Office Emergency Response Team on 3/5/17. The production fluid spill was reported to NERO at 7:45 AM on 3/6/17 and the motor oil spilled was reported at 10:28 PM on 3/5/17. Mr. Anderson and Mr. Bennett explained the following in reference to site activities and the spills. "Frac operations are in progress on the 6H well. Frac operations should be completed in about 2 weeks. The first spill occurred on Sunday morning 3/5/17 at approximately 3AM. 1/8 gallon of motor oil spilled to matting near tank battery. The d
2587334 Primary Facility 2017-04-21 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 11:50 in response to a reported spill of (3) barrels of flowback fluid to secondary containment on 4/20/17. The weather conditions were mostly cloudy to partly sunny skies and breezy with temperatures in the 50's. I met with Rich Cashen, site company man, completions foreman of Repsol. Mr. Cashen and I discussed site activities and the spill incident in the company man trailer. Mr. Cashen stated the following." We are in the the process of drilling out frac plugs on the 12H well, and flowing back the 1H, 3H, 4H, 5H, 7H and 8H wells. There are (14) plugs left to be drilled out on this well. The wells currently in the flowback phase of operations are sending natural gas to the gathering pipeline on the pad to the sales line. The 2H, 6H, 10H and 11H wells are currently shut in. The 2H and 11H were in production and the 6H and 10H have not yet been flowed back. The 12H well is the last well where frac plugs will be drilled out. The spill occurred at 16:00 hours on 4/20/17. 2596986 Primary Facility 2017-05-23 Routine/Complete Inspection Violation(s) Noted At time of inspection, arrived on site at 10:00a.m.

The well was recently completed and is in production. The well tag was on the well and correct. The cellar was about half full of water with no bubbling noticed. The (20x13) annulus vent pipe was open and venting into the atmosphere with no gas detected, the (13x9) annulus vent pipe was open and venting into the atmosphere with gas detected 100% LEL and peaked at 26% methane with no measurable flow, the (9x5) annulus vent pipe was open and venting into the atmosphere with gas detected at 100% LEL and 100% methane with no measurable flow. There was 1039psi on the production casing and 473psi on the production line. There was a crew on site picking up the containment from the recent drilling and stimulation activity. The 500bbl water tanks that were used during stimulation were still on site and on containment, the containment appeared to be in good condition. 2599609 Primary Facility 2017-05-31 Follow-up Inspection Outstanding Violations - Viols Req'd At time of inspection, arrived on site at 9:50a.m. for a follow up inspection for gas detected at the (13x9) annulus during an inspection conducted on 5/23/17.

I met with Jordon Blanchard from Control Tech, Nick Piet and Ryan Elsey from Repsol Oil & Gas. Jordon used a DP-IR gas meter that was calibrated on 5/12/17 to check for gas at the annuli. There was 0% gas detected at the (20x13) annulus. There was 8.82% gas detected at the (13x9) annulus. There was 100% gas detected at the (9x5) annulus. Nick said Repsol would continual to monitor the gas at the annuli and contact the Department if there are any significant changes. At that time, remedial action will be discussed if needed. I left the site at 10:35a.m.

2612874 Primary Facility 2017-06-20 Routine/Complete Inspection No Violations Noted Arrived at the site at 12:30. The weather conditions were mostly sunny skies and breezy with temperatures in the low to mid 70's. I met with Rich Cashen, Completions Foreman and Dave Williams, Water Management Coordinator, both of Repsol and Andrew Senefelder, Staff Environmental Scientist of Golder Associates. Mr. Senefelder is in the process of performing a post hydraulic fracturing and completions assessment. Repsol is breaking down containment and cleaning up the site. The assessment consists of spraying distilled water on the pad surface and utilizing a soil conductivity meter immediately after spraying the water to obtain conductivity readings of the pad surface. The intent of the assessment according to Mr. Senefelder and Mr. Cashen is to evaluate the condition of the pad surface where the containment was set in place and others areas of the pad relevant to completions activities. The assessment will also determine if hydraulic fracturing flowback and/or production fluid impacted the pad surface via
2622535 Primary Facility 2017-07-18 Routine/Complete Inspection No Violations Noted I arrived at the site at 12:55. I was accompanied by Frank Hoehle, PADEP O&G WQS during the inspection. The weather conditions were sunny skies with a light breeze, very warm and humid with temperatures in the mid to upper 80's. There was no activity or personnel at the site. All wells at the location are in production. Secondary containment utilized for completions operations has been removed from the site. During the inspection the Department performed specific conductance (electrical conductivity) screening of water on the pad surface, in well cellars, the production fluid tanks containment structure, the gas production units containment structure and frac tank containment structure. There were no elevated conductivity readings of concern. All readings recorded were less than (600) micro Siemens/cm. We were not able to locate a post storm Erosion and Sedimentation inspection report in the mailbox on the access road near the pad entrance. The Department left the site at 13:46.
2637036 Primary Facility 2017-09-06 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 12:54 in response to a reported spill incident of (2) gallons of diesel fuel and (4) gallons of transmission fluid. The weather conditions were cloudy skies, temperatures around 60 degrees, with calm winds. Rainfall has ended recently. I met with Steve Thompson, completions consultations of Repsol. Mr. Thompson stated the following in reference to site activities and the spill incident. "Currently, flowback of the Kropp 3H well is in progress. Over the course of the last week the 5H, 7H, and 9H wells were flowed back. The 1H, 10H and 12H are the next wells to be flowed back. The 2H, 4H, 6H and 8H are in all production. The wells on the Kropp pad were flooded out due to communication with some of the wells on the Repine pad during hydraulic fracturing (stimulation) there. Flowback is needed on this pad now due to the well communication issues. The spill happened on 9/6/17 at 00:40 hours, 12:40 AM. The spill occurred as a result of a vacuum truck hitting a cement block as the driver wa

The well was in production. The cellar was full of water and there was no bubbling noticed. There was a 500bbl tank on containment and the containment appeared to be in good condition. The tank was connected to the bottom of the separators. I took a walk around the location and there were no obvious concerns noticed.

2802803 Primary Facility 2018-11-14 Routine/Complete Inspection Outstanding Violations - Viols Req'd At time of inspection.

The well was in production. The cellar was full of water and over flowing onto the well site with a thin layer of ice and there was no bubbling noticed. There was no sheen noticed on the water and would suggest the cellar be emptied. The (20x13), (13x9) and (9x5) annuli were venting to the atmosphere. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2852950 Primary Facility 2019-02-15 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 11:04 in response to a reported spill incident of (0.5) gallons of production fluid. The weather conditions are mostly cloudy skies and windy with temperatures around 40 degrees Fahrenheit. Chris Hartman, well tender of Repsol reported the spill incident to the Department. I contacted Mr. Hartman to obtain the details of the spill. See the reporting/releases section in this inspection report for additional details.

I met with Jeffrey Blanton, well tender of Repsol, and Christian Mondi, Environmental Data Systems of Resource Environmental, Montrose, PA. Mr. Blanton informed me the following regarding the spill incident. "Spill was discovered around 3 yesterday, 2-13-19. A hammer union on the flowline on the 3H well leaked. Not sure how long it was leaking. There are no reports of prior leaking. We used absorbent pads to mop up the spill. The well was shut in, and the thread on the hammer union was sealed. The well is back on-line now."
