1067684682 2013-05-12 I was onsite to witness the cementing of the surface for the Putnam 5H well. While waiting on a broken down Schlumberger pump truck I checked the status of the 1H well. The well is currently capped and grated over in the cellar of Nomac 72. The cellar is full of water and the well appears to be static at this time.
1206773845 2013-04-30 I was onsite to witness the cementing of the 9 5/8 intermediate string that was set to 1938 feet. There were complete returns throughout the entire job and no issues seen. The cement was weighed at 15.7ppg before it was pumped down hole. A total of 20 barrels of good cement were returned to surface and the cement was scaled at 15.3ppg. Nomac 72 will wait 8 hours before tripping back into the hole.
1367173994 2013-04-27 I was onsite to witness the cementing of the 13 3/8 casing set at 605'. There was some issue with the casing being able to be set on bottom, the rig continued to circulate and was able to get casing on bottom in order to pump the cement to plan. The cement was scaled at 15.7ppg before it was pumped down hole. In total 20 barrels of good cement was returned to surface. The cement returned to surface was scaled with 5 barrels left of displacement and it weighed 15.4ppg. The plug held 500psi above bump pressure for 5 minutes and no flow back was seen on the cement truck.
1368411917 2013-05-10 I was onsite to witness the 5.5 production string set at 15700' be cemented. The casing was reciprocated and rotated throughout the job. The job was cementing according to plan. The floats held and 3 barrels were returned to the pump truck. Nomac 72 will immediately skid the rig the Putnam 5H well.
1481774144 2013-08-19 I conducted a site inspection of this pad in response to complaint #4423. Complainant emailed Heather Maher of the Carroll County Health Department. They are seeing discolored water, dark stains on the banks, sheen like gas on the water and big slime silicon that is very heavy but floats. Complainant states the frogs are almost gone, the fish are acting weird, the deer are not coming around like they normally do and the Blue Herron is not around. The residence is located near the drilling pad at 2246 Alamo Rd. SE Carrollton, OH 44615. The wells on the drilling pad have not been hydraulically fractured yet and are static with no annular pressures seen on any of the wells. The storm water outflow normally installed on the location has been removed and runoff water from the pad is no longer being discharged off the location. I also inspected the residence and talked with the landowner regarding his issue. I walked the pond and saw the
-1882311438 2013-04-29 I was onsite to check the status of Nomac 72 drilling their 12.5 hole. They are currently at 1375' and are slide drilling. They have started taking on about 200 barrels of water per hour from the point they hit the base of the Big Injun and the Berea Sandstone. They are ordering 4 more frac tanks on location to handle the influx of water. They expect to TD the hole tonight and cement tomorrow mourning.
1981866570 2013-08-12 I was onsite to check for the possibility of runoff from the drilling pad running into a local residence pond. The property and the ponds were not clearly identified in the report and I checked some in the area. The ponds appear to have no issues and the pad is currently dry and does not show any signs of substantial runoff. A clearing has been made near the pad for what looks like a pipeline right away. An inspection will occur more thoroughly when more information is received.
-2046779590 2015-01-14 I was on location to conduct a status check of this well. The well is open flow with a wellhead tree assembly. Jersey barriers were installed around the wellhead. The tank battery consisted of 8, 400 BBL production tanks and was diked. A windsock was installed on the top of the production tanks. Other production equipment on the well pad consisted of:
429085438 2013-03-28 I was on site to witness the cementing of the 20 conductor casing. My inspection revealed that Precision Rathole has drilled a 26 hole to a depth of 16' and finished drilling a 24 hole to a total depth of 110'. They ran 115' of 20 conductor (inside diameter 19.124) and had to cut off approximately 5' of the conductor casing. O-TEX cemented 110' of 20 conductor with 200 sacks of Class A 2% CaCl, 1/2 pound per sack cello-flake, and 0.4% C-41P. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.24 gallons per sack mix water. O-TEX pumped 42.03 barrels of cement slurry and displaced the cement with 35 barrels of freshwater. There was good circulation throughout the job and approximately 10 barrels of cement was circulated to surface. My inspection revealed no violations at this time.
719148652 2013-05-08 I was onsite to check the drilling of Nomac 72 in their 8.5 lateral. They are currently at 14,850' of a estimated 15,600' total depth. They expect to TD the well tonight/tomorrow mourning and the cement job could likely be Saturday. We also discussed the setting of the Intermediate casings in regards to the requirements of the permit. They had a question of exactly what approx 1950' on the permit meant and if they felt that the casing would be better set 5 feet short of that depth if they could set it at that depth if it was in 100% shale.
-898444843 2013-07-12 I was onsite to check the erosion controls of this wellsite following strong storms that made it through the region the previous week. There were no issues on the pad and all erosion controls are working properly. The wells on this site are currently shut-in as they are waiting for the completions crew to come and hydraulically fracture the wells. All the six wells have well caps on them and the cellars are watertight as they have not been pumped out since the rains came through. There are no violations at this time.
969767437 2013-07-03 I was onsite to update the GPS coordinates for this wells as well as check the annulus pressure of these 6 wells drilled by Nomac 72. It is unsure when the wells be hydraulically fractured at this time. Currently there was a vacuum truck on location sucking water off the pad. There was also a Stonebridge crew onsite removing the rig containment that was leftover. All the wells were checked and none showed any signs of pressures on the wells. The wells are currently static and there are no violations at this time.
