2509338 Primary Facility 2016-06-20 Complaint Inspection No Violations Noted On 6/20/16, I performed a complaint inspection at the Hill/Hopkins site and JM Sphar 4 well in response to complaint 320508 and made the following observations:

•Water samples were collected for SAC 946 analysis in response to the complaint. •Hill/Hopkins Site:

    -Site was in generally good condition with two wells drilled.
    -Containment appeared to be well maintained.
    -Vegetation on site was patchy in some areas (behind production units and on cut slope above impoundment).
    -The depression around well 2H was filled with water.
    -No evidence was found of spills or leaks on the site at the time of inspection.

•J M Sphar 4: The well and surrounding area were in good condition with no evidence of spills or leakage. Vegetation and berm around the well head and associated tank were also intact.

2511085 Primary Facility 2016-07-13 Administrative/File Review No Violations Noted Administrative review of lab results on 7/13/16 indicates that the water supply does not appear to have been impacted by oil & gas activity. Results and documentation for complaint 320508 forwarded for final determination.
