| An inspection of this well revealed the well head had fluid in its dike, and gas was bubbling out of the annulus.  I determined that it was gas by using my Sperian PHD 6 gas detector.  The tank battery dike was also full of fluid that had an oil sheen on it and when tested with high range quantabs, tested at 2.2 which is .  I spoke with Ron McBride at 11:11 am about these issues, and he agreed to come out later that afternoon and pump the fluid out of the dike, into a truck and haul it away.  He also made mention of moving the tank battery closer to the road.  We also discussed what needed to be done so that the annulus will no longer be venting gas.  He agreed to put a plug in it.

| A follow-up inspection revealed that the well has now been placed back into production.  Paul Grims service rig had been placed on well to perform a pump change.  One (1) 100 bbl tank with no knockout was taken from the battery on Maysville Williams Rd and placed at location.  The well was pumping at time of inspection.  Identification present at well and tank.  A fitting leak was observed on bottom of tank.  1 drip per seecond.  High Range Quantabs showed Cl- exceed maximum value of 10 units.  At 10:58 am, I called Mr. Doug Kitchen in regards to inspection.  Reminded Mr. Kitchen of Remedial Restoration for well site and lease road.  He states that Mr. Ron Mcbride is enroute to fix leak.  Mr. Mcbride arrived at location and fixed leak.  Also, discussed Remedial Restoration with Mr. Mcbride.  NOV for non-production has been resolved.

| A complaint was received alleging that the Parley and Lowell Nutter wells are idle.  According to the owner of Chieftain Energy, Mr. Doug Kitchen, and the landowner, Mr. Lowell Nutter all seven wells are tied into each other to produce house gasMr. Nutter states that little gas is obtained from these wells.  Mr. Kitchen admitted that a few wells need some work.  Mr. Kitchen will be given credit for 1 well since some house gas is generated. The other 6 wells must be plugged or produced.  Mr. Kitchen was given a verbal NOV on 10/18/2006.

| The Division of Mineral Resources Management received a complaint alleging non-production from this well.  The complainant states that the well has not been pumped for a few years.  An inspection of the well revealed that the well is idle.  The well contains a pumping unit with engine but, the valves were observed closed at the well head.  The flowline from the well was also found cut and not connected in two separate locations.  Approximately 50 feet from the well, black plastic from the tubing and casing side of the well head was observed cut just before the connection into a steel line.  The second location consisted of plastic and steel line cut underneath a bridge located on Maysville Williams RoadThe production records, in RBDMS, for this well were reviewed and showed that 1996 was the last year of production. It was also noted that the well had been transferred from Paul Grim to Chieftain Energy in 2001.  Moreover, Chieftain Energy is in violation of ORC 1509.11 for failing to submit a statement of production of oil, gas, and brine for 2007 and 2008.

As previously noted, on 5/20/2009, the well remains idle, as no action has been taken by Chieftain Energy to achieve compliance. The lease road will also need to be graded, due to erosion. Informed Chieftain Energy Contractor, Mr. Ron McBride, of the inspection results.

-1460508989 2012-03-22 An inspection of this well revealed the well head had fluid in its dike, and gas was bubbling out of the annulus. I determined that it was gas by using my Sperian PHD 6 gas detector. The tank battery dike was also full of fluid that had an oil sheen on it and when tested with high range quantabs, tested at 2.2 which is . I spoke with Ron McBride at 11:11 am about these issues, and he agreed to come out later that afternoon and pump the fluid out of the dike, into a truck and haul it away. He also made mention of moving the tank battery closer to the road. We also discussed what needed to be done so that the annulus will no longer be venting gas. He agreed to put a plug in it.

-1952376253 2009-11-25 A follow-up inspection revealed that the well has now been placed back into production. Paul Grims service rig had been placed on well to perform a pump change. One (1) 100 bbl tank with no knockout was taken from the battery on Maysville Williams Rd and placed at location. The well was pumping at time of inspection. Identification present at well and tank. A fitting leak was observed on bottom of tank. 1 drip per seecond. High Range Quantabs showed Cl- exceed maximum value of 10 units. At 10:58 am, I called Mr. Doug Kitchen in regards to inspection. Reminded Mr. Kitchen of Remedial Restoration for well site and lease road. He states that Mr. Ron Mcbride is enroute to fix leak. Mr. Mcbride arrived at location and fixed leak. Also, discussed Remedial Restoration with Mr. Mcbride. NOV for non-production has been resolved.

-2126262072 2006-10-18 A complaint was received alleging that the Parley and Lowell Nutter wells are idle. According to the owner of Chieftain Energy, Mr. Doug Kitchen, and the landowner, Mr. Lowell Nutter all seven wells are tied into each other to produce house gas. Mr. Nutter states that little gas is obtained from these wells. Mr. Kitchen admitted that a few wells need some work. Mr. Kitchen will be given credit for 1 well since some house gas is generated. The other 6 wells must be plugged or produced. Mr. Kitchen was given a verbal NOV on 10/18/2006.

-677305191 2009-10-29 A follow-up inspection revealed that Mr. Paul Grim has placed a service rig on this well. At time of inspection, rods and tubing have been pulled and will be steamed today. The pump has been taken to the Ohio Pump & Supply. Chieftain Energy is moving a 100 bbl tank from shared tank battery (Parley & Lowell Nutter tanks on Maysville Williams Rd) to a location near this well.
-756258423 2009-03-20 The Division of Mineral Resources Management received a complaint alleging non-production from this well. The complainant states that the well has not been pumped for a few years. An inspection of the well revealed that the well is idle. The well contains a pumping unit with engine but, the valves were observed closed at the well head. The flowline from the well was also found cut and not connected in two separate locations. Approximately 50 feet from the well, black plastic from the tubing and casing side of the well head was observed cut just before the connection into a steel line. The second location consisted of plastic and steel line cut underneath a bridge located on Maysville Williams Road. The production records, in RBDMS, for this well were reviewed and showed that 1996 was the last year of production. It was also noted that the well had been transferred from Paul Grim to Chieftain Energy in 2001. Moreover, Chieftain Energy is in violation of ORC 1509.11 for failing to submit a statement of production of oil, gas, and brine for 2007 and 2008.
