| Began trying to access the site at 10:15. Access is via Carlin Mining Rd, off Clarence Rd. Weather conditions at the time of the inspection were sunny and warm. No company representatives were present during the inspection.  Two wells had been drilled on this site: Carlin #3, and Carlin #3A, both had the same permit number. The plugged well with a candy cane vent, is the Carlin # 3 (based on a 2-14-06 inspection report – see copy). No site ID sign, well tag, or identification of any kind was observed at the plugged well. For the Carlin #3A there was a well tag (see photos).

Carlin # 3: an Altair 5X multigas meter (calibrated on 05/23/16) was used to monitor for the presence of combustible gas at 2” vent. No combustible gas was detected at the time of the inspection.

Carlin #3A: There were no vents or annuli to monitor for gas. No excessive corrosion was observed during the inspection. No violations were noted at the time of the inspection.  Off location at 11:15.

2488803 Primary Facility 2016-05-25 Routine/Complete Inspection No Violations Noted Began trying to access the site at 10:15. Access is via Carlin Mining Rd, off Clarence Rd. Weather conditions at the time of the inspection were sunny and warm. No company representatives were present during the inspection. Two wells had been drilled on this site: Carlin #3, and Carlin #3A, both had the same permit number. The plugged well with a candy cane vent, is the Carlin # 3 (based on a 2-14-06 inspection report – see copy). No site ID sign, well tag, or identification of any kind was observed at the plugged well. For the Carlin #3A there was a well tag (see photos).

Carlin # 3: an Altair 5X multigas meter (calibrated on 05/23/16) was used to monitor for the presence of combustible gas at 2” vent. No combustible gas was detected at the time of the inspection. Carlin #3A: There were no vents or annuli to monitor for gas. No excessive corrosion was observed during the inspection. No violations were noted at the time of the inspection. Off location at 11:15.
