| On 10/10/17 at 1414 hrs I conducted an inspection of the Spahr 22 well site. No one was present at the site. It was sunny and 75 degrees F.

The Spahr 22 well was drilled by Victory Energy in 2010; the target formation listed on the well plat is the Bradford Group. The well was transferred to Tenaska on 7/25/13. The well, pump jack, production equipment and tank were enclosed in a fenced area. The well was tagged. According to DEP records, the well is in production. The site is restored and permanently stabilized as per 25 Pa Code Section 102.22(a).

A Well Site Restoration Report (WSRR) was not found on file. A WSRR should be submitted as per 25 Pa Code Section 78.65(3), which states, in part:  "Within 60 days after the restoration of the well site, the operator shall submit a well site restoration report to the Department." It is encouraged that the WSRR be submitted electronically.

The well head assembly is a approximately 18" casing out of the ground with an approximately 10" casing inside of it with an approximately 7"" casing inside of it connected to an approximately 7"x 2" x 2 x tee style fitting with a 2" plug in one side of the tee fitting and a 2" production line connected to the side. Valves on  the production line were open.

The well was leaking gas and the emergency contact (724-349-6366) was notified of the situation. No other issues were noted and no violation were cited during this inspection.
2645878 Primary Facility 2017-10-10 Routine/Complete Inspection No Violations Noted On 10/10/17 at 1414 hrs I conducted an inspection of the Spahr 22 well site. No one was present at the site. It was sunny and 75 degrees F.

The Spahr 22 well was drilled by Victory Energy in 2010; the target formation listed on the well plat is the Bradford Group. The well was transferred to Tenaska on 7/25/13. The well, pump jack, production equipment and tank were enclosed in a fenced area. The well was tagged. According to DEP records, the well is in production. The site is restored and permanently stabilized as per 25 Pa Code Section 102.22(a).

A Well Site Restoration Report (WSRR) was not found on file. A WSRR should be submitted as per 25 Pa Code Section 78.65(3), which states, in part: "Within 60 days after the restoration of the well site, the operator shall submit a well site restoration report to the Department." It is encouraged that the WSRR be submitted electronically.

No violations were cited during the inspection. I departed the location at 1440 hrs. TLT

2752699 Primary Facility 2018-07-13 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. The well head assembly is a approximately 18" casing out of the ground with an approximately 10" casing inside of it with an approximately 7"" casing inside of it connected to an approximately 7"x 2" x 2 x tee style fitting with a 2" plug in one side of the tee fitting and a 2" production line connected to the side. Valves on the production line were open.
