No freeboard or E&S issues at the time of inspection. Conductor pipe was installed to 29 feet (9 5/8¿ pipe) and sanded in. Surface pipe was installed to 361 feet (7¿ pipe). Centralizers were set at 81¿, 166¿ and 274¿. Surface pipe was cemented with 65 sks of Class A with 6 bbls return. Water was encountered at 50 feet (10 gpm flow). Well current depth was 371 feet. Phillip Drilling just started drilling after the 8 hrs. Cement time. Phillip anticipates completion of well on 11/2/11 and move to power plant.

There was difficulty findining the site which resulted in the date of the inspection passed the spud notification. Both the Glass 8 and Glass 10 have an extended access road through the land owners property. Nearly the entire extent of the road was devoid of any BMPs. At the base of the road below the Glass 8, the road crossed a small creek. Because there were no BMPs installed, muddy water was leaving the road and entering the culvert and the creek. This was discussed with Doug Wadsworth (Keyrock) via phone conversation. He stated someone was to fix the problem asap. The well was drilled and frac'ed and in production. Site was very roughly graded. Pit had been removed. No stabilization measures were taken. The silt fence was comprimised. These matters were also discussed with Mr. Wadsworth. There was no well tag yet placed on the welll. Mr Wadsworth stated these were to be installed immediately.

2045689 2012-02-23 No Violations Noted Site inspection was compliance evaluation for the 12-28-11 violation.

Mike Buhite (Keyrock) was present during inspection. Access road had been rerocked with clean durable stone. Ditches along Glass 8 access road were rocked with hay bale water breaks. Stream crossing was stabilized with rock, hay bales and sediment fence. No GP-7 was obtained for the stream crossing. This permit must be obtained due to the construction of the stream crossing. Slopes were seeded and mulched. No further violations noted at this time. April Weiland, Water Quality Specialist PA DEP, was present during inspection.
