There is no packer between production tubing and intermediate? casing. Berm area down slope from this well contains free product and beyond this area there is an indication of stressed vegetation. |

Oil and a small amount of gas are being released from the annular space between the conductor and the surface casing. There is a trench leading away from the well roughly 3-4 feet wide and tapering off away from the well roughly 20 feet long filled with 1-2 bbl of oil. The well is located in a flood plane and according to the neighbor oil is released from the well and travels downstream on a regular basis when the river rises. | 1438002163 | 2015-07-24 | Orphan | No Violations | Today I inspected this well with Gene Chini, DOGRM Orphan Well Program Manager, to gather additional data and determine access to the well. We found the best way into the well was from the Nance property starting at 38.44504, -82.36848, which is a steep, narrow ATV path down into the floodplain. At the access point there is a sewer manhole, overhead electric lines, a fire hydrant, and telephone or cable lines. An access road will have to be installed and numerous trees may have to be removed to access the well. The well is actively leaking oil and has affected a 75 x10 area down gradient from the well. The well consists of 11 OD conductor casing, 6 7/8 OD surface casing (which protrudes 1.5 above ground level), and 2 3/8 diameter tubing on a hanger which is rusted off at ground level inside the casing. The 6 7/8 casing is open to the atmosphere and oil and gas are seeping out of the annulus between the conductor and surface casing. On the 2 diameter tubing there is a 2 tee with a line that goes to a regulator. There is no supply line connected to the regulator. This is assumed to be the former domestic service line. Above this Tee fitting is a gate valve that is attached to the 2 steel flow line. Above the gate valve the 2 tubing is swaged down to a 1 length of 3/4 tubing with a valve and a place for a gauge on top. There is debris piled up around the well and past inspections have noted oil marking the trees about 6 up from ground level due to past flooding events. There are no records for the well but information on the Ohio Fuel Gas map for this township shows that this well was drilled to the Ohio Shale with a total depth of 2832. Pictures taken, SEM is 239. |

The complaint was forwarded to the ODNR DOGRM Field Inspector on 3-14-2017. Upon arriving at 147 TWP Road 1142, crude oil odor was observed in the back yard. The well and oil release could be observed from the back of the house. An inspection of the well site revealed an area 30 feet by 50 feet with oil pockets and staining. There were oil rings on the trees at 2 feet off the ground. Total oil remaining on location is approximately 2 barrels. The South West Orphan Well Inspector was notified of the release and the complaint. The site has flood debris surrounding the well location. Paddy Creek is the nearest drainage for the site. |

Due to several recent heavy rain events, the well area was covered with water. I did not see any evidence of oil on the water and could not get close enough to confirm reports from previous inspectors and the landowner that oil does escape from the well when the area is flooded. The well is on the edge of a residential area off Holderby Road, in the Fairland subdivision of Proctorville. Access to the well will be from GPS: N 38.44406, W -82.36849. I talked to several neighbors before locating the actual property owner, Norman Nance. Mr. Nance owns 6 acres of this bottom land. We discussed the referral of this well into the Orphan Well Program and he requested that if and when plugging is completed that a gate be installed at the access point. I will have to return to the site after the water recedes to complete this inspection. Mr. Nance says in late summer, it will be dry enough to possibly drive down to the well. I supplied him with a Right-of-Entry form to fill out and return to our office. |

We walked the access road with the landowner and discussed what trees needed to be removed. He would like the tops taken out but will cut up all the wood except whats too big for the chain-saw. We will not put down gravel but will instead use mats to get over the wet spots in the road to be constructed. Water can be obtained from the Ohio River and they have their own cementing crew. The landowner stated that the road to be constructed to access the well can remain in place after plugging operations are completed but a gate will need installed at the access point. |
