17:10 on scene to find BBU Environmental with 1 vacuum trucks and a full crew working in the ditchline below the tank battery with some residual product migrating out of the bank below the tank battery. 18:00 an assessment was done and product was found migrating in quantities downstream, I requested multiple siphon dams be constructed and an additional water truck be deployed. With quantities of oil absorbent spill rags and booms deployed to stop the downward migration of oil. The oil was stopped at a distance of 1,917 down stream from the tank battery by the means of a siphon dam. 18:15 2ND water truck on scene and a 2nd assessment was done to determine the amount of product and flow pattern. 19:15 Aaron Wolfe with the OEPA has arrived and has been briefed on the clean up. 21:00 PM Product continuing to be recovered from the water downstream, spill rags beginning to be removed and the product being allowed to migrate downstream to the siphon dam recovery point. Operations are to continue throughout the night.

1125371281 2018-06-08 Production Wells No Violations On this day, I found the well equipped with 4.5 inch diameter casing and 2.375 inch diameter tubing with the production valves open. It is produced by means of a pumping unit with rods and an electric motor. The production facility consists of (1) 100 barrel steel tank, drip tank, and domestic meter all contained within an earthen dike. The production records are current and the tank is properly identified.
1430332047 2015-04-29 Production Wells No Violations Status check on the clean up process as a result of an oil spill on 04/28/2015-As of 12:00 pm 85% of the product has been recovered from the waterways and the 100bbl tank has been emptied and removed. Crews from Mountain State Environmental will finish removing the affected material from within the dike area and implement a water flush procedure to the affected area to allow the product to flow to a designated point of recovery. An estimated amount of 8 bbl of crude oil had initially escaped from the dike area with 65 bbl total recovered. Aaron Wolfe with the OEPA and myself have left the scene with the understanding we will continue to re-visit the site and communicate with each other during the remainder of the cleanup, with the company now left to finish mopping up the remaining product.
1432153347 2015-05-20 Production Wells No Violations Status check on the clean up process as a result of an oil spill on 04/28/2015- Upon inspection it was observed that the 100 barrel tank has been removed along with any contaminated soil. The siphon dams are still in place in the stream bed ditch line and small amounts of oil residue is still being recovered. Aaron Wolfe with the Ohio Environmental Protection Agency was updated on the clean up status on this day.
