Information per drillers log and cement job logs: Set 22' of 13 3/8" conductor and cemented with "A" containing 3% cacl2 & 1/4# flake. Mine at 275'-280'. Set 320' of 9 5/8" casing in 325' of 12 1/4" hole. Cement basket set at 240'. Cemented with 167 sacks of "A" containing 3% cacl2 & 1/4# flake (yield=1.18 cu ft/sk). Grouted backside at 07:17 05-20-11. Pit lined and freeboard maintained.

1977333 2011-05-31 No Violations Noted Well shut-in on 7" x 4 1/2" swage.

7" x 4 1/2" annulus closed. Pit lined and freeboard maintained.

1977501 2011-06-02 No Violations Noted Crews on-site. Frac¿ing in progress. Site ID sign and E&S plan located. Pit appears to be sound. There are no E&S concerns at this time.
1998545 2011-08-25 No Violations Noted Drilling appeared to have been completed, well was not equipped for production Observed no accelerated erosion or sedimentation in relation to the site. Site had been regraded and the vegetation had begun to grow, but did not as of yet meet the 70% uniform coverage criteria over the entire site. The operator should continue to monitor the site until well development activities are completed.
2051684 2012-03-12 No Violations Noted Well appears to drilled and equipped for production. Well apparatus and brine tank located on site appear to be in sound condition. There was no accelerated erosion observed on well pad or access road. The site is restored and meets the 70% vegetative coverage criteria. The Department notes that this well is not properly tagged as outlined in OGA 201(h).
