0163510929 2017-08-23 Plug / Plug Back No Violations The purpose of my inspection was to witness plugging operations the well was drilled to the Beekmantown Formation. Nicks Well Plugging, Alliance Company Man, and Appalachian were all on location. Nicks Well Plugging had their service rig rigged up over the well. A lined pit was located beside the well with a lined trench dug to the base of the wellhead. A water truck delivered 100 barrels of water and put it in the pit to use to load the hole. John Hudson with Appalachian Well Surveys rigged up on location. Appalachian tripped in hole inside the 2.375 inch diameter tubing. The total depth of the well was tagged at 6978 feet and a packer was recorded at 6700 feet in depth. After discussing with Alliance, Nicks Well Plugging, and Appalachian, the decision was made to shoot the tubing at 6670 feet in depth. Nicks Well Plugging pulled up a joint on the 2.375 inch diameter tubing to verify that the tubing was free. The tubing was successfully shot and free at 6670 feet. Nicks Well Plugging was experiencing pump issues on the pump truck so the bottom hole plug will be set tomorrow.

During my inspection I walked the lease road back to the plugged well site. The lease road and well site had been graded with vegetation established. The production equipment had been removed and no debris from an oil and gas well had been left behind. The location meets final restoration requirement written within the Ohio Revised Code 1509.072 (B).

Upon arrival following up on complaint id # 4968, I witnessed that the production equipment included a 210bbl tank and 60bbl polymer brine tank within an earthen dike. Also on site a horizontal separator with a 2 inch sales meter with a .25 inch orifice and a chart recorder. The well was equipped with 8.625 inch casing, 4.5 inch casing, 2 inch tubing, rods an Alten pumping unit with a natural gas 208 motor with a car muffler and is enclosed in a sound building which was running at the time of inspection. Photographs were taken, identification was present and production records were current.

1497963498 2017-06-20 Production Wells Violations Noted On the day of my inspection following up on Complaint ID 5629, I witnessed that the production equipment included a 210 barrel tank and 60 barrel polymer brine tank within an earthen dike. Also on site a disconnected line heater horizontal separator with a 2 inch salesmeter with a .25 inch orifice and a chart recorder. The well was equipped with 8.625 inch diameter casing, 4.5 inch diameter casing, 2.375 inch diameter tubing, rods an Alten pumping unit powered by natural gas with all valves in the closed position. Identification was present and production records were current with no production reported in 2015 and zero production reported in 2016. A plug permit issued on 8/19/15 will expire on 8/18/17. The well owner/operator shall submit a written plugging schedule to Doug Bennett at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

Nicks Well Plugging nippled up on the 2.375 inch diameter tubing and pumped 51 barrels of water with lost circulation material, and 53 sacks of Class A cement with lost circulation material, and displaced with 23 barrels of water. The rig tripped 6670 feet of 2.375 inch diameter tubing from the hole. Appalachian Well Surveys rigged up on location and tripped inside the 4.5 inch diameter production casing. The cement on the backside of the 4.5 inch diameter casing was logged at 6170 feet. The casing was shot off at 5200 feet. Nicks Well Plugging pulled up on the 4.5 inch casing and it was free.
