-1374755530 2012-07-09 On 7/9/2012, I conducted a production well inspection status check. My inspection found an oil spill approximately 20' long by 6' wide on the north side of the tank battery, I took photos of the contamined area, the well and the tank battery. I contacted the well owner, Robert Motz, to inform him of the spill and instructed him that clean-up operations needed to start immediately. Mr. Motz's brother, Chuck Motz, arrived at the site, where I showed him the comtaminated area. Additional violations include, the well site was overgrown with trees and weeds, the tank battery was overgrown with weeds and there was no identification posted. I gave Mr. Motz a clean-up deadline of 7/18/2012. I will perform follow-up inspections throughout the clean-up process. 9-9-02

1342019215 2012-07-11 On 7/11/2012, I conducted a follow-up inspection. I was contacted by Jim Vinecourt, who has been contracted by Mr. Motz to clean-up the oil spill. Jim told me that he would be using dumpsters from Enviroservice and that sampling of the contamination was conducted on 7/11/2012 to determine the proper disposal site.
1342457262 2012-07-16 On 7/16/2012, I conducted a follow-up inspection of the oil leak clean-up. The overgrown weeds and trees in the tank battery and around the well have been cut down. The oil leak was contained inside the tank battery dike, I took new photos. Mr. Motz's daughter-in-law, Vicki Motz, was working on getting a contractor for the clean-up, which should start by the end of the week. I will perform more follow-up inspections.
1342614293 2012-07-18 On 7/18/12, I stopped at 8 am to check on the progress of the oil spill clean-up. I received a call from Kleese Development Associates that said Mrs. Motz will be contracting them to clean the well site and remove the 210 barrel oil storage tank, with a possible start date of 7/18/2012. There is no activity at this time.
1342805293 2012-07-20 On 7/20/2012, I conducted an inspection of the oil spill clean-up by Kleese Development Associates. My inspection found all contaminated soil/gravel had been removed, the 100 and 210 barrel tanks have been removed, and the site was graded, seeded, and straw was spread. All clean-up operations are complete, inspection passed.
616326635 2012-07-19 On 7/19/2012, I witnessed the oil spill clean-up operation by Kleese Development Associates (K.D.A). The clean-up started at 930am and I arrived on site at 945am. The 100 and 210 barrel oil/brine storage tanks were drained and removed, the contaminated soil around the 210 barrel tank was removed. K.D.A will finish on 7/20/2012.
