Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION_COMMENT
-1161811337 2009-05-05 An inspection revealed no violations at this time. It was noted however, that the earthen dike walls would need to be built up to provide containment. More notably the front section was low as, a 5' x 10' bare area was observed outside the dike. High Range Quantabs (Titrators) were placed at this location and also inside the dike. The Titrators showed acceptable Cl- level. Contacted the company by telephone and recommended that the dike be addressed, when equipment is grading the lease road, on neighboring property, for the Patton #1, Permit No. 2553.
-1417258897 2010-10-12 An inspection was conducted to ensure compliance with Annular Disposal (AD) Revocation Order. Well head exposed and no AD connection was observed. Previous attempt to test well was not successful, as pressure would not hold per Mr. Mike McPeek with Kilbarger Investments.
2016430364 2005-05-18 Performed an AD mechanical integrity test on the well.
