-Pump jack not operating while on site. Talked with Ted Fleckenstein who farms the site for the landowner Zimmerman, and he indicated that he did not think the well was pumped in 2003. -Digital photos taken of the well and the tank. -Neither the well or the production tank had an ID tag. |

| 1095852912 | 2004-09-16 | -On site w/ Jeff Fry per complaint # 765 by Ron Zimmerman re: well & tank formerly on his father Robert Zimmerman's farm (now deceased). Farm was split 3 ways w/ tank on Ron's land & well on his sister Karen Bey's property. Ron is unable to get in touch w/ Mike Storms/North American Excavation, has seen no activity to the well, & power cut several years ago. Ron wants tank removed & well plugged on sister's property. Well & tank inspected 2/4/04 & N.O.V. # 1075997042 issued for failure to ID the tank &/or the well. Tank, now ID'ed w/ permit # "0069-ZRT, is about 200 bbls w/ about 2.5' of fluid. Small leak around the production line collar. Well is in unharvested cornfield. Ron given Mike Storms cell phone # & address on 9/17/04. . Jeff Fry indicated we will try to set up mtg w/ Mike Storms to view his wells. Will terminate N.O.V. for lack of ID. | ||||
| 1143730225 | 2006-03-15 | -On site to follow-up complaint # 765 received 9/7/04 from Ron Zimmerman re: wanting a tank moved that is on his property. The tank is on the west side of the old farmhouse formerly owned by his dad, Robert Zimmerman. When Robert died in 2001, the farm was split three ways among the siblings with the tank on Ron's property and well # 20069 on his sister's property. Mike Storms called several weeks ago and indicated he had moved the tank. Tank has been moved to around the 20069 well. Call made to Ron Zimmerman on 3/21/06 to follow-up on the complaint. Ron called back on 3/22/06 and did confirm the tank was moved but had a question regarding removing the line that used to run from the 20069 well to the former tank location that is still on Ron's property. Talked with Mike Storms on 3/23/06 and Mike indicated that in the next few weeks he intended to remove the old line. Mike said he had a portable tank he was planning to use to flush the line and would removed. Requested to Mike to call Ron to notify him that he would be removing the line soon. Coordinates of the new tank location was not gathered during the inspection due to muddy field conditions. Will GPS the new tank location in the next month or so and update the system. Reminded Mike Storms that he needed to produce the 20069 well as well as other wells he owns. |
