-1325658546 2013-10-03 I received a request from the landowner to meet at the site to go over the final restoration. I notified Derrick Petroleum and requested that they also have a representative present. On the day of my inspection I met with the landowner and Travis Dugan at the site. The landowner was not happy with the progress of the final restoration. Mr. Dugan had been on the property recently to re-seed and straw the area, per my request. The soil was graded out to the approximate original contour and the former tank battery site was covered in well established short grasses. The actual plug site was seeded with straw down and had no grass growth at the time of inspection. There were some miscellaneous items that need removed from the site. Travis Dugan loaded the items on his truck and removed them that day. The landowner agreed to leave the 2" buried along the course of the driveway. I notified the landowner that I will need to access the property at a later date to inspect the progress of the grass growth erosion control measures. The landowner request that the former well access road be left in place, as it currently being used to access a pond.
-1446166442 2014-05-20 On the day of my inspection I found that the former well site and tank battery site were approximately 85% covered with well established grass. I found no signs of erosion. This site passes final restoration. The former lease road is maintained by the landowner to access their pond.
-435834361 2013-05-03 On the day of my inspection Moran Well Service and Cemenco set a 200' plug at the top of the Big Lime; a 165' plug through the Berea; a 200' plug at the base of the surface casing and a 200' surface plug.

The area was seeded and strawed, but there was only about 30 - 40% coverage. There were no oilfield related materials or debris at the site. 1372862112 2013-07-03 On 7/2/13 I received a call from Dwayne Dugan of Derrick Petroleum to request that I inspect the progress of the final restoration on this site. On the day of my inspection I met Travis Dugan at the location to discuss the requirements of the restoration. The former well site and tank battery site were both graded out to the approximate original contour. They were seeded and covered with straw. The area was very wet, and there were visual signs of heavy deer activity on the freshly reclaimed area. I instructed Dugan to remove all oilfield related materials and debris, and to allow time for vegetation to start growing. The former lease road is being used by the landowner to access two ponds on the property, and Dwayne Dugan agreed to put more gravel down when they are finished with the restoration. This site does not pass final restoration at this time.
2016051490 2013-05-06 On the day of my inspection I found that Phil Moran had cut the casing off, capped it and buried it. The pit was closed and one stock tank had been removed.
