-1249193884 2011-09-27 Tank battery consists of one 100 barrel, one 210 barrel and a mechincal separator. Identification sign is posted. This is a rabbit well and it appears that work has taken place around well head. Flow line leak located 20 yards north of the wellhead. There is free crude oil (approximately 1 quart) at the site of the leak. Leak is in a wetland and there is freshly disturbed earth on top of the leak. Tested standing water east of the wellhead, Clorine = 87 PPM. Marker flag placed at rerlease site. Emergency number for the company called, (440) 693-4419. Message left including: contact information, location of the leak, warning that leak is in a wetland area, release is very small at this time, needs contained and repaired. Contact me with ETA.

1317224415 2011-09-28 Flow line continues to leak and the operator has not shut well down or repaired leak. Called emergency number and was given the pumpers cell phone number (440) 479-0760. Spoke to Jim Pavlic, the pumper for the well, who will go to well and shut in or repair as needed.

1894830528 2013-07-16 My inspection found the well to be equipped with a plunger lift unit. The well and tank battery are in good repair. The tank battery consists a 210 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition. The production of this well has not been reported for 2012. I spoke with the well tender Jim about the issue. The company contact information is not valid. NONEC
