-305993341 2012-06-20 Today I inspected this well. The well was not pumping during the inspection. Tall weeds are growing up around the tank and well. The tank did not have an earthen dike around it. The identification at the well is not legible.
-599178848 2012-04-24 At 8:24 AM, I called and left a voicemail message for Mr. Terry Helms requesting a return call.
326516981 2012-04-03 The Division received a landowner complaint alleging idle well on property for several years. An inspection revealed that the well was equipped with a pumping unit and tied into a nearby approx 150 bbl tank with no earthen dike. The well showed no signs of recent pumping. The identification displayed was not legible and improperly identified as Permit No. 3526. A review of the production reports for this permit number showed that the last year for commercial production was 2008. Note: Well owner reported 40 bbl oil from 2001- 2008 and mistakenly filed under the wrong permit number. A GPS reading was obtained for the well and tank battery location. On 4/3/2012, the well owner was notified by telephone of the inspection results and instructed to plug or produce well; if placed back into production provide new identification, build new earthen dike, and file statements of production. Mr. Helms states that well is not pumped in the winter time. The well location was researched and new Permit No. 62348 was assigned to this well. NONE
