-972570903 2014-08-06 On the day of my inspection accompanied by ODNR Inspector Justin Smith, I witnessed that the tank which was struck by lightning has been replace and clean-up is complete. Production equipment included a 210bbl tank a horizontal separator, a drip, a 2 inch sales meter with .75inch orifice and a chart recorder. Tank is shared with the 34059232920000 and 34059232940000. The well is equipped with 8.625 inch casing, 4.5 inch casing operated by a logic controller. Identification was present with production records being current.

1407032585 2014-08-02 On the day of my inspection I received a phone call from Chris Kimble of Red Hill Development stating that one of their tanks had been hit by lightning and that Antrim Fire Department had responded and the fire was out and everything was confined to the dike. Red Hill Development had workers there cleaning up the site. Upon arrival I found that a 210bbl tank which is shared with 34059232920000 and 34059232940000 had been struck by lightning and Antrim Fire Department was still on site with the fire out and Red Hill Development had a vac truck on site cleaning up the dike. I met with Antrim Fire Department Chief Don Warnock and he stated upon their arrival the tank was on fire and they used foam to put out the fire. I then met with Red Hill Development Supervisor Dave Gross and found that approximately 1bbl of fluid from the tank had spilled into the dike as a result of putting the fire out. Pictures were taken and all fluid was recovered inside dike. Monday Red Hill Development will return to replace tank and remove any soil contamination under tank and replace with clean fill.
