-288456306 2013-01-23 Upon inspection I found that the Pollution and contamination has stopped and is being cleaned up. The 2 line has been removed and a plug installed. The producer Will Wade is cooperating with my requests.
-876753058 2012-07-31 Inspection of the well head found the following equipment, 6.1875 casing, 2 tubing with a vent packer and .750vent packer string . Well is in the process of just having packer set. Inspector made contact with the well owner Will Wade and give him registration papers and asked him to get registered , Inspectors also met on site and asked for the tank battery to be fixed and proper well identification to be put up.

1358432882 2013-01-17 Compliance agreement and complaint # 3929 follow up Inspection found that the Producer by the name of Will Wade had built a new production fluid storage area as part of His compliance agreement. Upon inspection I found where a 2 drain line has been attached to a 250 gallon waste water tank and was buried and hid underground during the tank battery construction in such a way to allow the tank to be drained into a nearby stream. Inspector will note that the tank can be seen clearly from Sr 724 and I had drove by in the last 48 Hours and the tank was full and a fluid level with oil on top could be seen. Inspector will speak with His supervisor Rocky King on this matter.
1358566614 2013-01-17 Inspection report to document that verbal communication was made with the producer Will Wade in regards to the clean up of contamination caused by the intentional dispersal of production waste from a 250 gallon plastic tank through a 2 discharge line to a point next to the Elk Fork stream. Inspector Stolarik had been observing the site regularly and noticed that the tank had been drained and no obvious sign of where a water truck had entered the area was present and after further investigation a impact point was found next to the stream and the valve open on the tank. The producer Will Wade did verbally admit to draining the tank and thought He was ok to operate under the Mississippian exemption law. I explained to Mr. Wade that He had drained a unknown amount of oil out with the water and needed to start clean up immediately. Inspector Andrew Corder contacted Ken Metler with the initial notification of the spill and I followed up with Ken Mettler in regards to a recommended plan of recovery and clean up. Division Hydrologist Kyle Tharp and Supervisor Rocky King were also notified on this day.
1358568098 2013-01-18 Inspection in order to follow up on the spill recovery from the draining of a 250 gallon production waste tank. On this day I observed that the area affected by the spill had begun to be cleaned up by the producer Will Wade. Inspector will note that I did meet with Ken Mettler from the Ohio EPA while He was in the area on another matter and reviewed the spill report and recommended plan to clean up the spill and a mutual agreement was reached with both parties approving. With both parties being ODNR Inspector Stolarik and Ohio EPA Enforcement Agent Mettler.
1359555945 2013-01-30 Compliance notice follow up inspection found the contamination site clean up still in progress and in compliance with the Violation notice; The producer Will Wade has secured a Bond and is registering the well and other wells He owns in the direct area. Inspector will note that discussion has also taken place between myself and Will Wade in regards to cleaning up production related equipment not in use around the site.
1359797493 2013-01-31 Compliance agreement follow up inspection found that the pollution & contamination clean recommendations is being followed. Inspector will note that the general area around both the well head and the tank battery both need the miscellaneous debris left scattered about by the produce Will Wade need cleaned up. Contact was made with Mr. Wade on 01/20/13 and He was advised of his housekeeping issues and told to clean up his scattered well related debris in the area left on someone else's property. Mr. Wade has been working with registration trying to correctly register all of His wells with the division.
1363098630 2013-03-11 Inspection found that the producer Will Wade is working on cleaning up the area around the well site to remove the well related debri. On 03/12/13 I spoke with Erica Freeman from the Division's Regrestration section and Will has applied for and been issued a bond and is working to properly report His production.Initial Violations issued by Andrew Corder on 08/08/12 can not be resolved at this time. Initial Violation 255187563.
1365041809 2013-04-03 NOV follow up found Will Wade is Bonded and has provided proof of insurance. Mr. Wade has complied with my request in regards of any production related regulations but is still working on finishing ongoing site projects. I asked Will to purchase proper well identification tags.
