-2038663627 2012-08-29 I&O well leaking h2s gas . Complaint from Columbus ( Terracina Maxwell ) concerning a well which is leaking h2s. After checking well I found that thus well has been in the I&O file in Columbus for five years. Waiting for Columbus to plug this well.
1194451610 2007-04-27 Arrived on location and found well idle wellhead has fallen off, I cannot get in touch with complainant, Mr Todd rents this property and I told the renter to have mr todd get in touch with me.
1345926622 2014-03-19 Accompanies Inspector Norburt Lowder while witnessing plugging operations on this well. We witnessed the bottom hole plug being pumped. After it was pumped, the tubing got hung in the hole and the wire line company came out and shot the tubing. I also participated with Inspectors Justin Smith, LJ Parkins and Josh Garland in taking an H2S awareness class put on by O'Brian Safety Services. They were also there monitoring the site for H2S gas coming out of the well bore.
1395259000 2014-03-19 Accompanied Inspectors Josh Garland, Andy Thomas, and Justin Smith to this idle/orphan well site to attend an H2S safety training meeting with O'Brien Safety Services. After the training, we witnessed the attempted removal of the 2.5" tubing, which had become hung up inside the 4.5" casing. The tubing had been shot by Appalachian Well Surveys in an attempt to free it, but the attempt was not successful.
1395263914 2014-03-19 Witnessed portion of plugging operations. Tubing was hung in hole after bottom hole plug was set. A casing cutter was used in an attempt to free tubing at 4560 feet. Service rig still unable to pull tubing when I left.
1491380923 2014-03-18 I attended H2S training with O'Brien Safety Service. The well is being plugged by Moore well service. Inspector Norbert Lowder was the lead inspector.
2088193093 2014-03-19 I went out to this plug job for H2S training. The well was an idle orphan well that is being plugged by the state. O'Brien's Safety Services was on site for the H2S. They were giving the training while the well was being plugged.
34718564 2014-05-27 A follow-up inspection was conducted on 03/26/2014 for contract compliance.

The gravel driveway used to access the site has been finish graded. The access from the gravel drive to the well and the wellsite has also been finish graded and seeded and vegetative growth established. The visible piping, downgradient of the well has been removed. Final Restoration completed and approved.
