| 1163878391 | 2009-11-02 | Inspection revealed that the plugging pit was on fire and it burnt a couple of airlines and gauges on the control panel. The service rig operator advised that they had hauled the fluid from the plugging pit and were in the process of burning the pit liner at the landowners request when the pit caught fire. They got the fire out and we were discussing the situation when the Uhrichsville Fire Chief - Art Tripp arrived on site. He advised that somebody had called the fire dept and advised them about the fire at the well site. We advised him what had happened and then he left the site. No violations at this time. | ||||

| 314201315 | 2014-01-15 | I am on site today to check on the status of this well. I found that the well was plugged on 10/30/2009-- Brett Farnsworth was the Inspector at that time and he did not complete a plug report. Met with the landowner and he went with me to the site and I found a small slip which needs repaired before the final restoration can be completed. Will return this spring to meet with operator and landowner on site. I did contact Atlas office to advise of the restoration. | ||||
