1221676040 2007-09-17 Rig moving on site. Lease road is stable and constructed out of durable material. Called Cy from the company, requested that silt fence be installed around the stream crossing. Cy stated he will install silt fence and straw bails. Slope south east of the rig is vegetated. Tank battery is under construction west of the well. Pits are in clay operator had finished lining them before I was on site. Operator was anchoring the liner down at the time of inspection. Capstar rig one on site, will be spudding today.
1221842724 2008-09-18 On site for surface cement. Job went well, Berea top 270 Bottom 330. 431' of pipe set at 443 LJ. Pipe was new R-3 23lb pipe. Rig crew took good samples of Berea and shale. Pipe was set open ended. Class A cement 150 sacks, 10BBL return of good cement to surface. Cement fell back Berea was taking water. Requested that the operator tag cement and grout as needed. Operator has also cement CND.
1222195009 2008-09-23 On site for log of well. Well was TD at 4306', bit size 7 7/8. Pits are in good repair. Show of oil in the working pit. Operator is going to run long string and cement long string with 200 sacks of cement. 7 and 7/ 8 hole and 4.5 inch pipe is .2278 Cu ft per Ln ft. 200 sacks of cement/ 1.18 yield 200X 1.18= 360 yield. 360/.2278= 1580' of fill up. TD is at 4306' Top of the Newburg is 3552' cement needs to be 100' above. Top of the cement needs to be at 3452'. Bottom of the salt is at 3452'. 200' sacks of cement should get the top of the cement to 2726'. Operator grouted surface casing with 75 sacks of class A cement, cement had fell after surface.
1222207478 2008-09-23 On site for long string. Good circulation total of job. Plug down 5:49pm. Grouted surface at the end of the job, ran cement from the top, dammed off the ditch to allow cement to hold around casing.
285136287 2013-04-03 My inspection found the well to be equipped with a plunger lift unit. The annulus valve was open and plumbed into the production tank. The well and tank battery are in good repair. The tank battery consists of a 100 BBL production tank and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition. No violations found.
690297715 2011-07-26 Well is a rabbit well. Affected area has been reclaimed. Tank battery is in good repair, 210 Bbl tank and a mechincal separator. Area around tank battery and well head have been returned to pasture. Spacing is correct. Dike is in good repair.
