-1444620376 2011-08-24 An inspection of spring house revealed "foot valve" area remains slightly turbid. The area before leaving spring house (catch basin) was clear and has been cleaned. No drilling activity today. The well owner stated they only drilled in afternoon on Monday. The hardcoat was lost on drill bit.
-1487072792 2011-08-18 An inspection of the spring house revealed that the "foot valve" is now visible, as water has cleared since last inspection. No drilling activity today.
-1845288383 2011-09-09 An inspection of drilling operation revealed company placing 5.5", 17#, surface casing in wellbore. The casing will be cemented next week.
-694930788 2011-08-12 An inspection of drilling operation revealed 66' of 8" drive pipe and total depth of 80'. GPS reading obtained for the well. An inspection of the spring was conducted with the landowner, Mr. Himelrick. He states that from noon to about 3 pm yesterday the spring was "rauled up." Around 6pm, it began to clear up a little more. Mr. Himelrick would like for the spring to be cleaned after we think its going to quit getting rauled up. Mr. Himelrick further states that he gave the well owner/driller, Mr. Kevin McQueen the bills from the first day (ie filters).
-748236556 2011-09-23 At 8:10 AM, I received a call from Mr. Kevin McQueen, the well owner/driller. Mr. McQueen states that clay has settled and will wash additional sacks on backside of 5.5" casing. To date, 15 sacks of clay have been washed on top of cement.
1210770869 2011-08-01 On 8/1/11, I received this complaint from Mr. Will Ziegler. The complainant was contacted at 8:58 AM and a message was left requesting a return call. The complainant returned my call at 3:33 pm and states that the drilling of Permit No. 6053, Himelrick #7 had "mudded up" his spring on 7/28/11 and actually got worse on 7/29/11. The complainant described water as tannish brown color on 7/28/11 and unable to see through water on 7/29/11, The well owner/drilling company, Mr. Kevin McQueen spudded with cable tool drilling rig on 7/25/11. The complainant states approx 50' of drive pipe had been set on 7/29/11. As of today, the complainant states that his spring is just fine and has cleared up. The complainant is concerned about future disruption to this spring from oil and gas drilling, as it is the sole source for human consumption and his livestock. The complainant states that drilling rig is idle and owner/driller on vacation this week. There were no changes in taste or quantity observed by the complainant. This complaint was documented in the Oil & Gas Complaint database and forwarded to Oil & Gas Hydrologist, Kyle Tharp for investigation. Complaint ID# 3253.
1317210033 2011-09-12 I witnessed the cementing of the 5.5" surface casing. No circulation or cement returns were observed at the surface. Note: cement company states good lift pressure. Instructed well owner that remediation would be required. Notified West Region Oil & Gas Supervisor, Will Ziegler.
1414649764 2012-07-24 An inspection revealed that the Preliminary Restoration meets ORC 1509.072. The well is situated on a location designated as an agricultural field.
1621400781 2011-09-13 An inspection of the springhouse at 8:25 AM revealed no turbidity. A fishing line with a small weight was ran on backside of 5.5" casing for attempt to tag cement, but unable to get past possible collar at 19'. The well owner would start welding seal between 7" and 8" casings to prevent contamination to spring from the surface. Note: two options had been given-place material on backside or weld seal.
2133535661 2011-08-17 An inspection of the spring house with Mr. Kevin McQueen, well owner/driller, was conducted prior to washing clay on backside of 7" casing. The spring showed turbidity more notable in area containing the "foot valve". Witnessed the washing of 5 sacks of clay and allowed to settle. Returned to the spring and noted the same conditions as previously mentioned.
30274277 2011-09-22 Witnessed the washing of 11.5 bags of clay on backside of 5.5" casing to ensure protection of freshwater. The clay will be allowed to settle and washed as needed until clay is observed at the surface. The annular space between 7" and 8" casings contains a rod which has been welded to seal any void and prevent contamination from the surface. Two empty clay bags were rolled and placed around the backside of 8' casing and dirt placed on top to prevent any clay displaced from backside of 5.5" casing into backside of 8" casing.
419092617 2011-09-01 An inspection of the spring house conducted with Mr. Kyle Tharp, DMRM Hydrogeologist revealed that spring water has cleared. A sample of this spring was obtained by Mr. Tharp. No drilling at time of inspection.
45048810 2011-08-17 Arrived at Mr. Himelricks property and conducted an inspection of the spring house with Mr. Kevin McQueen prior to washing clay into annular space between 7" casing (24#) and 8" drive pipe. Note: 5.5" casing will be ran 50' below USDW and cemented to surface once this depth has been reached. The water was not clear and showed signs of turbidity. Washed 5 sacks of clay and allowed to settle. Returned to the spring house at 12:30 PM and noted that status remains unchanged. Clay was not observed in the spring house. The well owner/driller and landowner have agreed that spring house would be cleaned once spring is no longer disrupted.
61469148 2011-08-16 An inspection of the drilling operation revealed 91' of 24lb 7" casing placed inside the 8" drive pipe. Tomorrow, clay will be washed into the annular space between 7" and 8" casings to protect the freshwater spring and prevent any further communication. Note: The surface casing will consist of 5.5" casing ran below the Big Injun Sandstone to approx 365' and cemented to surface. The well owner/driller, Mr. Kevin McQueen has cleaned the spring house.
714314110 2011-08-11 An inspection of the drilling operation revealed wellbore contains 49' of 8" drive pipe with total depth of 73'. The well owner/driller made 10' today and will check the status of landowner's spring.
