-959317476 2013-05-15 My inspection found the well to be equipped with a plunger lift unit. The well was shut in. The well and tank battery are in good repair and are surrounded by a locked chain link fence. The tank battery consists of a 100 BBL production tank, a 50 BBL brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition. Charlie from Enervest was on location to start selling the well.
1235412246 2009-02-21 Called from Inspector Bob Worstal, 7:00am, oil release at the Flaherty-Hauser #2 well. On location 8:30am. Range Resources personnel on site: Woody Mchaniels, Carl Shafer, Doug Horning, Joe Duuall, Carl Day, Jim Smith, John Fleming W Pol Contracting personnel on site: Wade Pol, Mark Rhode, Kevin Ludwig, Jim Sutliff, Ryan Bissuri. ODNR personnel on site: Tom Hill, EPA personnel on site: Jim Irwin, Bainbridge Fire Department personnel on site at the time of clean up: Wayne Burge. Land owner was also on site. Top of the lubricating box was knocked off of the well. Lubricating box was in the well cage, rabbit was not located. A mixture of crude oil and rust was sprayed from the well. The plume of sprayed material extended 500', north and northeast and was approximately 150' wide. Amount of release was approximately 5 gallons of material. There was fresh snow on the ground, that showed the fine particles that were released. There were no puddles of released material. Pictures of the area were taken. Operator constructed a pit on site and lined the pit. Affected snow around the well head was removed and placed into the pit for temporary storage. Affected ice from the stream was also removed and placed in the pit. Operator constructed a three stage siphon dam in the stream, down stream of the release. Booms were also installed in the stream. Snow west of the stream had a very fine mist of particles on it. Operator lined the west side of the stream with straw bails to absorb any potential run off from the affected area as well as a portion of the east side of the stream. Stream drainage controls were approved by the EPA. Release was caused by unpreventable equipment failure. Company immediately respond to release. No NOV to be issued.
1243203430 2009-02-25 Called Bainbridge zoning before inspection, invited them to attend inspection. Mike Joyce and Shane Wrench on site during inspection. Operator has filled clean up pit. Half tank on site at the tank battery, half tank has material that was cleaned up during the spill in it. Silt fence has been put up west side of the stream. Siphon dam and booms are in place. Three booms noted during inspection, two before the dam and one down stream of the dam. Site will need to be graded and seeded when weather permits.
165074847 2012-09-28 I inspected this well at the request of Columbus and I found no annular disposal being used at this time. Well head and tank have the proper ID displayed. This well shares a tank battery with the Flaherty-Hauser #1 (Permit # 21113). The tank battery is diked and fenced. The tank pad contains two mechanical separators, 1-210 barrel, and 2-100 barrel stock tanks. The well head is fenced and is operating on a plunger lift system. No violations.
