-1745991868 2013-03-28 My inspection found the well to be equipped with a pump jack. The annulus valve was closed. The well and tank battery are in good repair. The tank battery consists of two 210 BBL production tanks, two 100 BBL brine tanks, and two mechanical separators. The proper identification is located on the tank battery. The lease road is in stable condition. The lease road and tank battery are shared with permit number 2-2055.
1278962295 2010-07-12 Rig has moved on site. Operator has completed digging the pits. Work pit has 6 inches of water in the bottom of it. Operator will pump pit dry and mix gel into to the bottom of the pit before lining it. Approximately a half inch of water at the south end of the blow pit, operator didn't dig blow pit as deep. Operator will be need to solidify pits when closing. Operator pumped pits down mixed gel into the sides and bottom of the work pit. Operator is using 6 mil liners.
1279191382 2010-07-13 On site for surface cement. Hole #1 drilled to 40 feet hole was 17 inch. Operator set on joint of CND 42 feet R-3 32lb pipe. Pipe was set in the hole. Operator hit the Sharon Conglomerate less than 20 feet deep. Operator drilled hole #2 to 421 feet. Berea top was 300' bottom 330', show of oil in the Berea. Berea samples had an petroleum odor to them. West end of the work pit had oil floating on the surface. Operator took good cutting samples, operator had samples segregated on a board in 10 foot increments, samples Berea, coffee shale and additional gray shale's were on the board. Operator was conditioning the hole while waiting on cementers. Fresh water used for cementing was tested, pH was neutral, Cl 980 ppm. Petroset broke circulation ran cement, displaced and 11BBLs of good cement returned to surface. Cement held at surface, hole was static. Operator set 390 feet LJ of 8.625 R-3 20lb pipe. Inspected BOP, threads appeared to be in good repair, bladder appeared to be in good repair. Air body, threads appeared to be in good repair. New rubber was on the rig floor to be used during drilling. Operator had a new 7 7/8 Reed bit on the floor for the hole.
1279220486 2010-07-15 Drill depth 2529' operator is drilling on fluid. Operator used 6mil no reinforced liners. There are tears in the liner at the top of the berm. Advised rig that fluid needs to be kept well below the tears or liner needs replaced. Owner will be advised that 6mil liners shouldn't be used. They are not sufficient. Operator was advised to keep dust down. Operator is currently pumping water into the blow line to keep dust held down. Site is in good repair, operator is using diapers to clean up oil and grease spills under the rig. Lease road is stable. Permit is posted.
1279541891 2010-07-18 On site for long string job. Operator ran used 10.5lb pipe. Operator ran 7 centralizers on the long string. Operator broke circulation conditioned hole. Water used for cementing had a pH of 8. Petroset Cementing broke circulation, ran two bags of gel ahead, ran cement, displaced cement. During cement displacement Petroset lost water flow from the 300BBL tank, Petroset was able to maintain circulation with water from their tank until flow from the 300BBL was restored. No lose of circulation during cementing. During drilling, the Newburg made water. Pits will be pumped down Monday, there was 2 feet of free board in the pits. There were tears in the pit liner approximately 1.5 feet above the water. Review of the Geo-log; Berea top 196' bottom 244', Lockport top 3542'.
1396977889 2011-07-26 Well is a pump jack well. Affected area from the drilling has been reclaimed. Well spacing to battery is correct. Battery spacing; separator to production tank 9'5", spacing between tanks is correct. Tank battery also has permit 22055 produced into it. Identification signs for both are posted. Identification sign is posted at the well head. Dike is in good repair.
1837356491 2011-03-21 Tank battery is in good repair. Well is a pump jack well. Site has been graded and seeded. Vegetation is starting to become established. Will inspect for Preliminary Restoration latter in the spring, to allow for further establishment of vegetation. Lease road is stable to tank battery.
338839583 2010-07-14 On site for BOP test. Pressured BOP to 600psi pressured hole to 200psi. No lose of pressure after 15 minute test. Pits were in good repair. Site was stable, drainage controls were in place. Lease road was stable. Fire extinguishers were in place. The extinguisher at the end of the dog house, inspection tag had been updated. Permit was posted in the dog house. Worker safety signs were in place. Hand rails were in place. BOP had not been chained down at the time of BOP test, operator wanted to make sure it passed before chaining it down, incase it needed adjusting.
