INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION_COMMENT
-136721053 2012-04-30 At the time of my inspection the cable tool rig was set up on the well and 5.5" bit was ran to about 400' until an unknown obstruction was encountered. The crew is now setting up to run tubing in and circulate hole.
-1939794195 2012-05-16 At the time of my inspection the well was topped off from 48' to the surface with 28 sacks of Sackrete.
-1955094967 2012-05-02 Rain in the area delayed continuatiun of claenout till mid afternnoon. At the time of my inspection the 2" tubing was at 452' and continued to 545'. The tubing was to hang up and is getting worse as they go deeper.
-376435608 2012-05-01 At the time of my inspection a new piece oc casing had to be welded on existing 8" to attach a wellhead. 2" tubing was then ran to 400' to circulate oil out of the wellbore. The tubing is starting to hang up on what we believe is a few remaining pieces of 6" surface casing around 270' (bottom of the Big Lime).
-511940394 2012-05-03 At the time of my inspection another obstruction was hit at 745' and tubing continues to hang up not only on the way down but on the way out as well. Continued circulation has produced no more oil from wellbore.
-587709463 2012-05-17 At the time of my inspection the plate has been welded on the top of casing and buried four feet below grade. The pit is also being closed and field tile is being replaced.
1337181753 2012-05-04 Plugging Contractor is Black Swamp Oilfield Services, Mike Coyer. Plug set at TD of 744 due to the collapsing shale formation and a crooked hole. The drill pipe is catching on pieces of the surface casing still left in the hole from when it was removed. See Plug Report for details.
1337184796 2012-05-15 On site for setting of plug 2 from 342' to 48'. Cement pumped through 2" tubing with circulation of cement to surface. Fresh water formation is taking some cement so level is dropping. See Plug Report for details.
2012623642 2012-12-04 At the time of my inspection the area was cleared of all equipment and all pits were closed. The area was then roughly graded for farm use next year. Final Restoration Passed.
