1997788 2011-08-17 No Violations Noted Well sign and E&S plan was located along access road. No E&S issues and no IW issues during inspection. First Energy was contracted to install well from Dorso. Tool pusher is Josh Detweler. Conductor pipe consisted of 13 3/8﾿ (sanded) to 39 ft. Surface Protection was 7﾿ to 928 ft. with 200 sks of Class A 3 % CC ï¾½ flakes with 12 bbls return to surface. Current depth was 2730 ft. Production string (4 ᄑ﾿) anticipated depth of 3578 ft. Water recorded at 2﾿ at 170 ft. No freeboard issue at the pit.
2013846 2011-10-20 No Violations Noted Site visit was follow-up from previous violation inspection on 8-16-11.

Site had been regraded and seeded/mulched. Ditches were still muddy and experiencing some erosion. Sed traps and filter sock were catching most of the sediment. Permanent stabilization techniques (ie rock lined ditches, durable rocked road) should be implemented when permanent restoration takes place. Well head was marked with id number. Well was bubbling at surface. Information was provided to DEP inspector. No violations noted at this time.
