2084232 2012-07-19 In Compliance with Policy Well inspected as a follow-up to an incident on 07/18/12 which resulted in the production fluid tank located onsite catching on fire. Met onsite with operator representative Doug Manners. CONSOL Gas Company personnel disconnected the well after the fire was extinguished on 07/18/12. Fire was reported shortly after 3PM 07/18/12, to CONSOL Gas Company and by 4PM CONSOL GAS Company had contacted the Department. Fire did not appear to have affected the ancillary well equipment located onsite. Tank and diked area had been covered by plastic to prevent precipitation from collecting in the diked area. There was no evidence observed to indicate that the production fluid released to the diked area had made its way outside of the diked area. Well had been disconnected from the flow line and production tank and shut in. McCutcheon is tentatively scheduled to be onsite at 8AM on 07/20/12 to begin site remediation.
2091871 2012-08-01 No Violations Noted Well inspected as a follow-up to an incident on 07/18/12 which resulted in the production fluid tank located onsite catching on fire. Brine tank had been removed and the affected diking material had been excavated and contained in a covered roll-off box located onsite. The well was still disconnected and not in production. At this time the Department is requesting that the operator forward the following documentation no later than 09/22/12: 1) Sample results showing all affected media had been exacavated, this should include background sample results to compare the excavated area sample results to. 2) Documentation outlining how the incident began, at time the incident was reported to the Department, and what actions have been undertaken to remediate the well site.
