Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION_COMMENT
2091337 2012-08-07 No Violations Noted TOP HOLE DRILLED. CEMENT AND CASING INFO. LOOKED GOOD. WAITING ON HORIZONTAL RIG TO COMPLETE DRILLING OF WELL.
2101907 2012-09-19 No Violations Noted DRILLING OF WELL COMPLETE. SITE LOOKED GOOD AT IME OF INSPECTION.
2127628 2012-11-09 No Violations Noted Check to see if drill cuttings pit greater than 20 inches above seasonal high groundwater. Some evidence that it is but will recheck when pit empty and liner gone.
2127657 2012-11-27 No Violations Noted On November 27, 2012, I stopped at the Tharp site to check the status of the cuttings pit; I talked to Jeff Kerr. They were emptying the pit at inspection without drying the cuttings. The plan is to fill in the pit to expand the pad for future fracking operations.
2128122 2012-12-06 Violation(s) Noted On December 6, 2012, I stopped at EQTï¾’s Tharp site; I talked primarily with Bill Balog and Rex Nelson, both are Construction Specialists for EQT.

EQT had finished removing the liner and I wanted to see if there were signs the pit was originally constructed within 20 inches of seasonable high ground water. They scrapped the pit down to bedrock. The bottommost part appeared to be mostly fractured rock. It became more like soil in the top 2 feet of the pit. Most of the material below 2 feet deep was wet and there was a small amount of free water in the bottom of the pit. About 24 inches from the top of the pit, you could see a layer where the soil changed. The soil below that was very dark and wet, this area smelled like muck soil. It appeared dryer above this. Based on the soil report submitted by EQT, the observations and photos of the seep I took on November 9th and the conditions I saw in the pit on December 6th I believe that EQT built the Tharp cuttings pit in the water table. This is a violation of ᄃ78.56(a)(4)(iii).

657130 78.56PITCNST - Impoundment not structurally sound, impermeable, 3rd party protected, greater than 20" of seasonal high ground water table
2128183 2012-12-13 No Violations Noted On December 13, 2012, I stopped at the Tharp site and found that the pit was completely filled in and they were starting to use the reclaimed space for vehicles and trailers.
2131640 2013-01-16 No Violations Noted First visit, gather data took samples. Final determination on lab results.
