2044925 2012-02-13 Violation(s) Noted Arrived at the site at 11:20 AM in response to a production fluid spill that was discovered by a GDS associate on February 8, 2012 at 13:25 PM at the Fraser Tank Pad. The quantity of production fluid spilled was determined to be approximately 20 barrels (bbls). The spill occurred as a result of structural damage to a manway door on the side of a Baker tank. Production fluid leaked out of the tank on the northwest corner of the Pad and traveled approxiamtely (41) feet south and then into and through an (4) inch PVC pipe undedrain. The fluid then daylighted and exited on the fill slope for approximately (30) feet and then changed direction and headed north and on the Laser Pipeline ROW for (71) feet. Please refer to Fraser 1 Surface Activities Inspection Report for further details. 632572

2052434 2012-03-13 Outstanding Violations - No Viols Req'd Containment liners are still in place on the excavation of the spilled material in the northwest corner of the pad and fill slope and Laser Pipeline Right-of-Way. Sampling results are pending. The orange construction fence that was set in place around the perimeter of the excavation on the Tank Pad was damaged. Currently, there are no frac tanks stored at this location. I used an Extech, Exstik (2) pH, conductivity, Salinity and TDS meter to filed screen an area of ponded water on the containment liner on the fill and trench were the underflow drain is located. Conductivity readings were in the general range of (2) milliSimens/cm.
