2223464 2013-11-06 Violation(s) Noted On November 6, 2013, I stopped at Chesapeakeï¾’s James Herold B pad for a routine inspection. This site has two gas wells associated with it, 8H (059-25922) issued 2/14/12 and 3H (059-25990) issued 5/2/12. The associated ESCGP-1 is ESX11-059-0011.

There was no sign at all identifying the site and I had to ask residents where the site was. Additionally, I found no well permits, E&S plan, PPC plan or any other paperwork on-site.

The site itself is only partially developed. There were perimeter controls (silt sox) and some dirt piles but it was obvious that nothing had been done at this site for months. I saw no evidence that any well was spudded. The access road was rutted and starting to form gullies.

The 3H well permit expired in May 2013 and since this site does not have an active well permit, this site should have been restored by June 2013. Further complicating things is that some of the soil from James Herold A is stored on this site.
