Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
1120444559 2010-11-08 Inspection on Friday found Gordon getting fracted
1282740450 2010-08-24 Tom Rouse called to say he had surveyor re-shoot location - confirms that it is 300' from property line and 410' from Ludlow's Berea well, #63810. Too close to produce Berea.
1290444403 2010-11-22 Inspection this day found all but one frac tank gone - ship tank flowing back into it - talked to J & L and Jeff said they hauled 2 loads of flow-back water - went to Virco well. Should have been more water hauled but examined the ground and found no evidence of dumping. Ground very torn up from the timbering, dozer has worked most of the soil where tanks sat so believe some of flow-back water probably dumped but can't prove it. Sampled water still laying in ruts and found no evidence of brine. Service rig still on well, just plumbing it up and jack not set yet. Tank connected to well, still needs ID's.
-1368883583 2010-08-10 Inspection this day found pit has not been emptied, no tanks on location for fracting - I was told fracting was scheduled for 8/13/10 but don't look like they will be ready by then. I have serious doubts, based on GPS reading, that well is far enough from property line, have call in to Columbus to advise what to do. Called Tom and left a message to not spend more money on well until we confirmed in was legally drilled. 8/12 - Talked to Steve O. in Columbus and he feels well is legally drilled, anyone who feels it is not should hire a surveyor to confirm. Roma is not allowed to produce the Berea - only option would be to buy and plug #63810.
-1413725868 2010-09-13 Inspection this day found well has been perfed, not fracted. Operator should be doing that soon. Pit has still not been filled in - deadline is pasted. Operator says needs to leave pit open to run rods, tubing and will close immediately.
1638629255 2010-11-01 Talked to Tom of Roma - says plan to frac on Friday afternoon, 1st zone. Rob Armstrong in charge of water - will re-use his water so don't have to worry about dumping until the end. Wanted to use pit to flow back into but told him his pit liner wasn't good enough - would have to fix if wanted to do that. Talked like he was going to do the Berea and I told him he cannot - well too close to old Berea well. Had told him this before but he must have forgot - told him I would double check with Columbus - they have again said NO to Berea production.
623865380 2011-08-24 Time for reclamation to be done. Talked to Tom - he says knows it is time, just got last logged out and will have service rig on well in next week or 2, then can finish. Pit is filled in, black plastic still showing above ground, frac tank still on location and they are storing brine in it. Warned him not to drain brine (I believe he is but no proof), and to get ID's up. Entire property looks pretty bad due mostly to logging but also drilling. Will give him another month or 2, then write NOV - said he is going for the Fall grass seeding. Gas meter shows he is selling gas. Well has jack, needs more dike. Cannot pass PRD this day.
-97161287 2010-11-06 Inspection this day to see if flow-back water going into tanks. Well had stopped flowing but had it plumbed up to go in tank. Waiting on perforators
