1031143929 2014-04-25 I received an email from Mary Ann Stumbo in regards to a suspected orphan well in her client's back yard. She requested an inspection to confirm the identification of the well. I found a cinder block planter filled with potting soil that had well tubing extending approximately 3 feet into the air. The tubing was capped and I did not observe any signs of fluid leaking, or the odor of natural gas. I took GPS coordinates and photographs. I contacted ODNR Orphan Well Inspector Cindy VanDyke to report the well. I did not find any records of this well in the ODNR database. I submitted my data to Steven Riley to generate a new permit number. I requested that the potting soil be removed to get a better look at the well casing.
-1086554929 2014-05-13 On the day of my inspection I met with ODNR Orphan Well inspector Cindy VanDyke at the well. We took photos, and measurements, drew a sketch, filled out a site evaluation matrix and got a right of entry (ROE) form filled out by the home owner.
885264070 2014-05-07 On the day of my inspection I found that the potting soil had been removed from the planter. The tubing led into 8 or 9 casing. There were no fluid or gas leaks observed. I took more photos.
