-1050177024 2010-07-12 Lease road is rocked to the tank battery. Lease road through the corn field has a drainage channel north of it, the channel shows signs of erosion, channel will need to be monitored. Annulus on well is plumbed into the flow line. The line has two one way check valves on it. Tank battery and wellhead each have identification signs. Spacing in the battery is within 1509 standards. Two mechanical separators, two 100BBL brine tanks and two 210BBL tanks in the battery. Tank battery is lined. Operator has a drain pipe in the dike, pipe has a riser on the end in the battery and capped at the discharge end.
1203522213 2008-02-20 I performed a rig inspection. I GPS'd the well. The drilling depth is 1670'. The drilling pits and pit liners were in good shape. There were no violations at this time.
-1343145579 2011-08-25 Annulus is plumbed into the production line. Took annulus pressure at valve one foot of off the annulus. Annulus had 150psi on it. Well has 368 feet of 8 and 5/8 set. There was bubbling on the out side of the 8 and 5/8 inch pipe. Tank battery is in good repair. Lease road has gully erosion forming on the north side, operator will need to complete corrective actions to repair and prevent erosion. Operator will be contacted.
1894142720 2008-03-06 Drilling of well has been completed. Lease road has been graded. Lease road has a rill erosion channel forming on the south side of it. Operator will need to complete reclamation as soon as weather permits. Operator has installed silt fence south side of affected area. Silt fence is in good repair. Pits have been closed on site.
-294007720 2011-08-30 Well is in production. Production casing pressure of 120 PSI was taken from gauge on the production line. Spoke to Dave Harker in regards to annulus pressure. One-way valve may be stuck. He will have pumper check on well. Dave stated that this well usually has between 70 and 80 psi on it. Gas is from the shale. He will have the problem corrected. Allowable pressure on the annulus: 368 feet of surface casing X .303 X .70 = 78 PSI.
408863390 2013-10-17 My inspection today found a 300 BBL tank by the wellhead. The annulus pressure is 35 lbs. The 4.5 casing pressure is 45 lbs. The company has a service rig scheduled to service the well. No other changes since my last inspection.
955144516 2013-04-03 My inspection found the well to be equipped with a pump jack. The annulus valve was plumbed up into a pop off valve. The well and tank battery are in good repair. The tank battery consists of two 210 BBL production tanks, two 100 BBL brine tanks, and two mechanical separators. The proper identification is located on the tank battery. The lease road is in stable condition. The lease road and tank battery are shared with permit number 2-2111.
