1351433823 2013-12-30 On the day of my inspection I was on a follow-up from December 27, 2013 when an internal line ruptured on a line heater. I was notified that repairs and clean-up was completed at 7:20 pm on Friday December 27, 2013 upon arrival I witnessed that that repairs were completed and well was back on line. Contaminated stone as requested was removed and replaced with new stone.
1362144178 2013-02-26 I am on site today to witness the running and the cementing of the 26 inch diameter casing string. Express Drilling has drilled a 32 inch hole to 110 ft and set that same depth of casing. Stingray Cementing Service has cemented the casing with 280 sks and had 18 bbls of good cement returns.
194859188 2013-12-30 On the day of my inspection the site passed preliminary restoration according to 1509.072A.
48795341 2013-12-27 On the day of my inspection I was requested on site due an internal line in a line heater rupturing and antifreeze boiling over out of the top of the heater. Approximately 1 barrel of antifreeze was spilled on to the pad. Gulfport was instructed to pump remainder of the antifreeze out of the line heater and remove existing line heater and excavate down below contaminated stone and replace with clean stone. Contaminated stone is to be placed in a roll off box and disposed of properly.
919763818 2013-03-12 I am on site today to witness the running and the cementing of the 18-5/8 inch diameter casing string. Yost Drilling Rig # 7 has drilled a 24inch diameter hole to 370 ft and has installed 362 ft of casing. Stingray Cementing Service c/w 580 sks of class A -15.6 lb cement. Circulated 30 bbls of good cement returns to surface.
