-1123329685 2007-07-09 On site regarding a complaint received by Wayne Schalk DMRM supervisor. Complaint was that there is a gas leak at the well. Inspection findings, there is a small leak on the well head. I was able to hear the leak and feel it around the first hammer union below the rods. I called Dave Hill and left a message on his voice mail that there is a leak at the well and the fitting needs tightened. Site has been seeded and mulched. Vegetation is beginning to become established. Once vegetation is more advanced silt fence can be removed. Tank battery is in good repair. Two 110 BBL tanks and mechanical separator on site. Charcoal filter and lighting arrestors are in place. Lease road is stable and in good repair.
1159459923 2006-09-28 Drilling was active. Operator had constructed a ground pit for cement overflow. Pit water level was at ground level. I instructed drill operator to lower the water level in the pit. Operator is having tanks brought onto site. Ground pit will be pumped dry. No identification sign was posted at site. Permit was on site.
1163614347 2006-11-15 Operator is currently fracturing well. Trucks entering and exiting the site have been dragging mud onto the road. A complaint was received by phone. Called operator regarding complaint. Equipment was on site cleaning roads at the time of inspection. Operator will be bring a load of stone, for the lease road.

-1738150329 2006-09-26 Status check inspection to check on the steel drilling pits. The mud tanks, shale shaker, and cuttings tank were leased from Union Drilling. I spoke with Kilbarger's tool pusher and asked him to place a liner under the cuttings tank.
1893017353 2013-07-15 My inspection found the well to be equipped with a pump jack. The annulus valves is open. The well and tank battery are in good repair and are surrounded by a locked chain link fence. The tank battery consists of a 100 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The production tank is vented through a 55 gallon charcoal filter and up into a spark arrestor. The proper identification is located on the tank battery. The lease road is in stable condition.

554775833 2006-11-15 Complaint of mud on the road. Went to site, operator was actively cleaning roads at the time of inspection. Operator was scrapping the road with a plow and picking up large clumps of dirt. Suggested to operator that they bring in a fresh load of stone for the apron to help clean mud off trucks.
