1857630024 2009-06-04 A complaint was received by the DMRM alleging that 100 bbl tank is leaking. An inspection revealed that the earthen dike contained fluid approximately 6-12 in some areas with organic material on surface. The fluid directly around the two 100 bbl tanks was 2-3. Old oil was observed along the sides, back end, and between the tanks. The knock-out tank was not connected at time of the inspection. The 100 bbl tank with knock-out contained a section of weathered oil around the midsection to top. High Range Quantabs placed in the fluid revealed no chlorides. The well contains a pumping unit with an engine. All valves were observed open with attached flowline, as polish rod shows signs of pumping. The tanks appear to be empty at this time. Load line valve open with 3 line routed over hill. Valve closed at end of line with no observed leaks. The identification at the 100 bbl tank showed Johnson Oil & Gas, as the well owner instead of Mike Johnson.
-396055477 2006-09-07 An inspection was conducted at the facility owned by Mr. Mike Johnson on 9/7/2006. During the inspection, it was noted that identification was missing.
867469971 2004-02-26 An inspection of the well revealed no ID at the well or tank battery, oily soil in dike around production tanks, dike does not meet S.P.C.C. requirements
