-1383175203 2013-08-26 My inspection found the well in production. The well was equipped with a plunger lift unit and a domestic gas line. The tank battery contained a 100 barrel production tank, a 50 barrel brine tank, and a vertical separator. There were plugs on the load lines. There was no safety equipment on the production tank. There were chain link fences around the tank battery and well with locks that did not work. The dike met containment requirements. A 16 day production chart was located at the separator. An identification sign was properly posted. The lease road was in good condition. I called Todd Sutko at 330-506-2531. I instructed him to install the safety equipment, an identification sign at the well, and new locks.
1404927390 2014-07-09 I was on location to perform an urban preliminary restoration. I found during my inspection the site graded. Vegetation was well established. The well was in production and equipped with a plunger lift unit. The tank battery consisted of a 100 barrel production tank, a 50 barrel, poly tank and a separator. There was a dike surrounding the tank battery. Identification signs were properly posted at the well and tank battery. Locked, chain link fences surrounded the well and tank battery. The lease road was in good condition. All drilling equipment was removed. All debris was removed. The pits were closed. Urban Preliminary Restoration passed.
