Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
1598451175 2013-10-06 I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Express Energy Services has drilled a 26 inch hole to a total depth of 120 feet and ran 20 inch conductor casing to a depth of 120 feet. Allied cemented the casing with 236 sacks of ClassA cement (2% CaCl and 1/2 pound per sack floseal). The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.2 gallons of mix water per sack. Allied pumped 51 barrels of cement slurry, pumped 39 barrels of freshwater displacement, and there was 14 barrels of good cement returns to surface. My inspection revealed no violations at this time.
-184019674 2013-11-22 I was on location with inspector Kevin Perry to inspect drilling operations of Nomac Rig #73. They are preparing to rig down and move to the Lucas drilling pad. All equipment is on contaiment. No violations.
-1844606580 2015-01-13 I was on location to conduct a status check of this well. The well is open flow with a wellhead tree assembly. Jersey barriers were installed around the wellhead. The tank battery consisted of 6, 400 BBL production tanks and was diked. A windsock was installed on the top of the production tanks. Other production equipment on the well pad consisted of:
