1) multiple loads of gravel were being hauled onto site to surface access road. road was gravelled to about 300' beyond first existing gas well. grader and roller following closely behind gravel trucks. 2) road and cut sides of road are compacted with no signs of erosion. sloped sides of road are bare 3) culverts are in place in multiple spots along road. Currently exposed soils are at the inlets and outlets of these culverts 4) Site ID sign and E&S plan are at entrance off of S.R. 3016 company asked to ensure proper installation of BMPs

Drilling is completed, and the well is shut in. Cellar is filled with water with a layer of ice on the top. Well head is leaking from somewhere below the surface of the water. Gas is bubbling up through the water all around the wellhead. There was no discernable smell of natural gas, but the gas could be heard bubbling. Pictures and a video were taken of the well head leak. A spill was noted between the well head and the pit. This spill appears to either be drill cuttings, or drilling mud. Spill will be turned over to WQS Sam Jones.
