2075571 Primary Facility 06/08/2012 Complaint Inspection Violation(s) Noted Inspection result of several complaints reported to the department about concerns with the amount of runoff and erosion caused by well access road ( N 40 46 19.5 W 79 06 01.2) which comes off of Brady Road. Complainants expressed concerns that if an emergency vehicle needed to access their lane; they do not believe it would be possible due to degradation in roadway.At the time of inspection, department noted accelerated erosion down gradient from access road entrance on private lane. It appears that due to steep nature of access road and failure to properly installed and maintain access road bmp's and/or properly stabilize; ultimately accelerated erosion and sedimentation did occur. Please note, department was unable to drive a 2010 Dodge Dakota up access road due to safety concerns as a result of scouring and sedimentation issues along roadway. Spoke with Larry Fetterman PDC Indiana office, would send a crew out. Dept conduct follow up
2078437 Primary Facility 06/21/2012 Compliance Evaluation No Violations Noted At the time of inspection, department immediately noted the addition of shale to the entrance to the shared access road. It appears operator has "rocked" the entire entrance eliminating ruts and accelerated erosion, although does not appear that operator added rock to steep portion of access road. It is recommended that operator add additional rock to access road in order to reduce the potential for accelerated erosion and sedimentation issues. It appears that operator has adequately responded to telephone call and subsequently notice of violation. At this time no violations are noted.
2513854 Primary Facility 2016-08-15 Complaint Inspection No Violations Noted Routine Partial Inspection. At time of inspection access road developing minor ruts. Shared access road that leads from Twp road to well access road is rocked, no E&S issues observed. It appears a small amount of sediment has washed onto the Twp road downgradient of shared access road. Department unable to determine if sediment is coming from shared portion of access road. Department recommends Operator continue to monitor entire access road to avoid any E&S issues.

Department attempted to contact Operator by phone. Department left message for Operator to return call.

2513861 Primary Facility 2016-08-17 Administrative/File Review No Violations Noted Admin inspection to inspection record #2513854.

Department contacted Operator by phone and left message to return call. Operator returned call later that day. Operator Rep Larry Fetterman stated he would send someone to visit the site to inspect the access road and see what needs to be done. A follow up inspection will be conducted at a later date.

2519633 Primary Facility 2016-09-08 Routine/Partial Inspection In Compliance with Policy Routine Partial Inspection to inspection record #2513854.

At time of inspection no E&S issues observed on shared portion of access road. No E&S issues observed on well site access road. It appears the Operator has installed 3 broad-based dips across the access road to control water run-off. This BMP appears to be an adequate TEMPORARY measure to control rain water. However, due to the steepness of the access road, the Department recommends Operator refer to E&S manual for information on permanent BMP's such as water deflectors and/or open-top culverts. A follow up inspection will be conducted at a later date to ensure BMP's are working correctly.

2530898 Primary Facility 2016-10-24 Follow-up Inspection In Compliance with Policy Follow up Inspection to inspection record #2519633.

At time of inspection no sediment observed on Twp road downgradient of access road. Erosion gulley beginning to form on private driveway. Gully begins beyond access road intersection, continues down driveway, and onto shared portion of access road. It appears first diversion ditch on access road is not being maintained. It appears ditch is directing some but not all water off of access road. Department recommends Operator continue to monitor and maintain access road to avoid any E&S issues. As per above mentioned inspection record, Department again recommends Operator refer to E&S manual for information on permanent BMP's such as water deflectors and/or open-top culverts. A follow up inspection will be conducted at a later date to ensure BMP's are working correctly.

2533402 Primary Facility 2016-11-02 Administrative/File Review In Compliance with Policy Admin Inspection to inspection record #2530898.

Department contacted Operator Rep Larry Fetterman by phone to request update on access road BMP's. As per above mentioned inspection record, at the time of that inspection the diversion ditch appeared unmaintained. Mr. Fetterman stated he would talk to the foreman who visited the site and follow up with Department. A follow up inspection will be conducted at a later date to ensure BMP's are maintained and functioning correctly.

2543907 Primary Facility 2016-12-08 Administrative/File Review In Compliance with Policy Admin Inspection to inspection record #2530898.

Department contacted Operator Rep Jeff Risenger by phone to request update on access road BMP's. Mr. Risenger stated the foreman visited the site and cleaned out the diversion ditch. The foreman also seeded and mulched the area to provide additional stabilization. A follow up inspection will be conducted next week to ensure BMP's are maintained and functioning correctly.

2544131 Primary Facility 2016-12-12 Routine/Partial Inspection No Violations Noted Routine partial inspection to inspection record #2530898.

At time of inspection no sediment observed on township road down-gradient of shared portion of access road. No E&S issues observed on shared portion of access road. It appears the first diversion ditch on the access road has been repaired and appears to be functioning correctly. The diversion ditch and surrounding area appears to have been seeded and mulched. Department recommends Operator continue to monitor and maintain access road to avoid any E&S issues. A follow up inspection will be conducted to ensure the access road BMP's are functioning correctly and being maintained.

2605171 Primary Facility 2017-06-19 Routine/Complete Inspection No Violations Noted Well appears equipped for production.
